This study sought to evaluate the long-term effectiveness of the Bacillus Calmette-Guérin (BCG) vaccine against tuberculosis (TB) in healthy, vaccinated adults in Khartoum, Sudan. The study included 100 participants who underwent tuberculin skin testing. Results found that over half (61%) tested negative, suggesting diminished vaccine effectiveness over time or cell-mediated immunity against TB. Males on average showed slightly larger induration than females. The study concludes that further research with larger samples is needed to better understand BCG vaccine efficacy in this population and help inform public health strategies against TB.

Screening of BCG Vaccine Efficacy among Healthy Vaccinated Adults in Khartoum,
Sudan
TITLE
Summary: A live attenuated vaccine known as Bacillus of Calmette-Guerin was created by two
French scientists to defend against the infection of tuberculosis (TB), a highly contagious illness
that is regarded as a major worldwide health problem. A Tuberculin test may be used to show
prior TB exposure before immunization. By screening healthy, immunized adult volunteers in
Sudan's capital city of Khartoum, this study sought to ascertain the BCG vaccine's effectiveness.
This study included a total of 100 (n=100) healthy participants. 55 (55%) males and 45 (45%)
female volunteers made up the group. In addition, majority of them had scars at the site of their
immunization and were older than 20. Only those who were clear of TB symptoms were
included and subjected to the manteaux test. Each participant underwent the manteaux test by
receiving a 0.1ml intradermal injection of the pure protein derivative PPD. Induration was then
assessed 48–72 hours later, and the diameter was quantified. The findings revealed that only 39
(or 39%) of the 100 subjects who underwent screening for the Manteaux test had a diameter
smaller than 10 mm, whereas 61 (or 61%) had a diameter larger than 10 mm. Six of the 39
positives (or zone 15 mm) and 33 of the 39 positives exhibit readings between 10 and 15 mm. 53
of the 61tuberculin test-negative participants had no post-PPD induration, and the remaining
participants had reading zones between 5 and 9 mm. Additionally, the outcome reveals that just
16 (41.03%) of the 39 positive participants were female, with 23 (58.97%) of them being men.
Men 13.96 3.29 have a higher mean zone reading among positive participants than women 13.81
2.22 do. The study found that more than half of the participants tested negative for tuberculin,
which could mean that either the vaccine was no longer effective at the time of immunization or
that the participants' cell-mediated immunity to TB has been diminished. Furthermore, there may
be a physiological reason for the difference in zone reading averages between males and females.
To clarify the findings, additional research should be conducted with a larger sample size and
utilizing a more sophisticated technique (IFN measurement).
Reaction: The study of screening BCG vaccine efficacy among healthy vaccinated adults in
Khartoum, Sudan holds significant importance in healthcare for several reasons. Firstly, it can
provide insights into the long-term effectiveness of the BCG vaccine, which is crucial in regions
with a high frequency of tuberculosis, like Sudan. Understanding how well the vaccine protects
adults can inform vaccination strategies and potentially reduce TB transmission. Secondly, the
study may identify any declining immunity, prompting booster shots, if necessary, which can
help prevent disease recovery. Ultimately, by shedding light on the BCG vaccine's efficacy in
this population, the research can contribute to more informed healthcare decision-making,
leading to improved public health strategies in the fight against tuberculosis in Sudan and
potentially serve as a model for other countries facing similar challenges.
In conclusion, the study of BCG vaccine efficacy among healthy vaccinated adults in
Khartoum, Sudan is a valuable endeavor with the potential to enhance healthcare outcomes by
guiding vaccination policies and ensuring continued protection against tuberculosis in a region
where it remains a significant public health concern.
