To apprehend the wanted person named Joseph P, Macapagal, male, 26 y/o, married,
laborer, and resident of Purok, Iglesia, Banga South Cotabato, with standing warrant of arrest
for the crime of homicide. Docketed under Criminal Case # 145-13 with recommended bail of
Php 100,000 for his temporary liberty, issued by Judge Khristina O. Montilla of Regional Trial
Court Branch 14, Sto, Nino South Cotabato.

SUBJECT : Final report re-shooting incident that resulted to death of Juan Dela Cruz

2. That on November, 10, 2023 at about 10:30am, this office received information that Juan
Dela Cruz were shot by an unidentified suspect in front of their house in Purok, Rizal 2, Sto
Nino, South Cotabato.

3. SOCO team recovered evidence from the scene. The team conducted deep investigation and
find out primary suspect. With that Preliminary Investigation, Questioning and Interrogation of
the suspect has done. Through Questioning and Interrogating the suspect admit his guilt, and
found out using illegal firearm.

4. The suspect was one Jhon P, Ciabo, 25 y/o, single, track driver, and a resident of the said
purok. The crime happened due to personal issues and intentions.

5. A case for Illegal Possession of Firearm under arms Act A case for Murder under Revised
Penal Code (RPC) of article 248.

2. in connection with above reference, please be informed this station had requested SOCO for
Ballistic Examination on the recovered fired cartridge of 45 cal. & awaiting for result. Coordinate
with the owner of the CCTV near the crime scene to check Cyber Crime 8 requesting photo
enhancement.

On or about 10:30am November 09, 2023, ACOP, ICPS attended meeting with cot. Daryl C,
Ong Cmdr, 102th Be PA & Lt. Jonny C, Sins for joint security measure.

 Intel Operative of this station are continuously gathering information for possible
identities& arrest of the perpetrators. Hot Pursuit operation still min effect.

What: Shooting Incident

When: November, 10, 2023 at about 10:35am

Where: Purok, Rizal 2, Sto,Nino South Cotabato.

At about 10:30 in the morning of November 09 2023, a shooting incident transpired in front of
the house of victim in Purok, Rizal 2, Sto, Nino, South Cotabato.

According to the witness, the identified suspect shot the head of the victim 2 times which result
to his death.

The victim was identified as one Juan Ponce, 24 y/o, married, driver, and a resident of Purok,
Rizal 2, Sto Nino, South Cotabato. The witness stated that the suspect was wearing Red t-shirt
and blue pants together with his gun. The victim was brought to the nearest hospital at Luntao
Medical Center in Sto, Nino Suth Cotabato.

The victim was identified as one Jake V. San, 29 y/o, married, micanic, and a resident of Purok,
uno, Banga, South Cotabato. The witness stated that the suspect was wearing blue t-shirt and
blue pants together with his gun. The victim was brought to the nearest hospital at Pinggoy
Medical Hospital Koronadal South Cotabato.
