Care Manager Adults - Job Description

Job Title: Care Manager Adults

Hours: 35 hours

Salary

Responsible to: Registered Manager

Overall Purpose:

To manage, co-ordinate, develop and promote all Crossroads Care South Thames current
and future short breaks/care services for the carers of adults with mental health needs, long
term health needs and those with physical and learning difficulties ensuring that Crossroads
clients receive a reliable and efficient care support service

Principal Duties and Responsibilities

Service

1. Process referrals and undertake all initial client assessments for all adults with mental
health needs, long term health needs and those with physical and learning difficulties
appropriately and promptly allocating these to the relevant Senior Care Support Worker/s as
appropriate once assessed

2. Be specifically responsible for the services for adults with mental health needs, long term
health needs and those with physical and learning difficulties processing all referrals
appropriately and promptly

3. Be aware of local services for the carers of adults with mental health needs, long term health
needs and those with physical and learning difficulties and their carers, in the communities in
which South Thames operate and act as a resource for clients and care staff

4. Operate the assessment, reassessment and care planning process for adults with mental
health needs, long term health needs and those with physical and learning difficulties within
Crossroads Care South Thames guidelines ensuring that client needs are matched with
CSW competences.

5. Operate and contribute to the development of systems which improve service delivery and
quality

6. Ensure that Crossroads Policies and Procedures are implemented, in particular where these
relate to service delivery and best practice

7. In conjunction with the Care Management Team participate in and implement a duty rota/on
call system

8. Attend client reviews and case conferences as required

with physical and learning difficulties waiting list within Crossroads Care South Thames
guidelines

Personnel

16. Be responsible for the management of a team of Senior Care Support Workers and through
them the management of a team of care support workers

17. Ensure that the allocation and co-ordination of the service best meet the needs of the client

18. Organise regular monthly meetings with Senior Care Support Workers and Care Support
Workers.

19. Undertake individual supervision and appraisal for the Senior Care Support Workers and
provide ongoing support on a day-to-day basis.

20. Monitor the supervision and appraisal systems for care support workers

21. Be responsible for and undertake the first stage of the complaints and disciplinary
processes within Crossroads Care South Thames

Management:

22. Be responsible to the Registered Manager for maintaining the service within the hours
budgeted for

23. Compile and maintain accurate records, service data and reports on service, submitting
these as required for monitoring purposes internally to Crossroads Care South Thames as
appropriate

24. Evaluate and monitor service quality assurance and standards of care including undertaking
spot checks to monitor CSWs work

25. In conjunction with the Registered Manager support the planning and delivery of Support
Worker recruitment, induction and in-service training including the NVQ programmes.

26. In conjunction with the administrator manage the computerised and manual systems for the
maintenance of accurate and confidential client records

Development:

27. Lead on the development and implementation of new projects for adults with mental health
needs, long term health needs and those with physical and learning difficulties

28. Identify and be aware of the particular needs of carers and utilize that knowledge to
increase awareness of those needs.

29. Develop effective relationships with the lead service managers for adults with mental health
needs, long term health needs and those with physical and learning difficulties

General:

1.7 Unless the nature of the position allows Crossroads Care to ask questions
about an applicant's entire criminal record we only ask about "unspent"
convictions as defined in the Rehabilitation of Offenders Act 1974.

2.0 TRAINING

2.1 We ensure that all those in Crossroads Care who are involved in the
recruitment process have been suitably trained and have adequate support and
guidance to enable them to identify and assess the relevance and circumstances
of offences. We also ensure that they have received appropriate guidance and
training in the relevant legislation relating to the employment of ex-offenders, such
as the Rehabilitation of Offenders Act 1974.

3.0 INTERVIEW

3.1 At interview, or in a separate discussion, we ensure that an open and
measured discussion takes place on the subject of any offences or other matters
that might be relevant to the position. Failure to reveal information that is directly
relevant to the position sought could lead to withdrawal of an offer of employment.

3.2 We make every subject of a CRB disclosure aware of the existence of the
CRB Code of Practice and make a copy available on request.

3.3 We undertake to discuss any matter revealed in a disclosure with the person
seeking the position before withdrawing a conditional offer of employment.
