CERTIFICATE OF NON-FORUM SHOPPING

I, PLT KARL DANIELLE Q QUIOCHO, of legal age, bonafide member of the


Philippine National Police, presently assigned at RMFB, NCRPO and designated as
Officer Duty (OD), RHQ, NCRPO, Camp Bagong Diwa, Lower Bicutan, Taguig City from
8:00 AM of January 8, 2024 to 8:00 AM of January 9, 2024, after having been duly
sworn to in accordance to law, do hereby depose and state:

That, I am filing an administrative complaint against the below named PNP


personnel for Less Grave Neglect of Duty pursuant to NAPOLCOM Memorandum
Circular No. 2016-002 Rule 21, sec. (b) para (1), sub para (d)” fail to comply with any
lawful order or instructions of a superior officer or the Chief of Police”, who were absent
during acounting of RSSF NAZARENO 2024 on January 8, 2024 @ 2:00 PM and @
4:00 PM held at NCRPO Grandstand, Camp Bagong Diwa, Lower Bicutan, Taguig City,
namely;

or pending before any other administrative discplinary authority. I undertake to inform
such fact within five (5) days such notice to this office or to the discplinary authority
where the original complaint has been filed.

held in front of NCRPO Grandstand, Camp Bagong Diwa, Lower Bicutan, Taguig City.

I have voluntarily executed this affidavit to attest the veracity of the foregoing
facts.
