15 Previous Europe Visa if any need date of issue

16 Date of Biometrics used for previous visa
(ignore if point 14 not applicable)

Europe with number of days in each country

Documents required to process application online before submission:

1. Passport front and address page copy


2. Letter from Current Institute
3. Bank statement of last 6 Month (E-statement would suffice)
4. Invitation Letter from Spain
5. Soft copy of photo colour/white background

Documents required to carry at the time of Biometrics/Submission:

Mandatory Documents:

1. Original passport valid for at least Six months with minimum 2 blank pages. (face to face)
The passport should be issued less than 10 years ago.

2. Two photocopy of Passport photo page and address page of the applicant.

3. Photocopies of all valid US, U.K. and previous Schengen visas.(if any)

4. One application form & undertaking form with all columns filled (We will provide)
5. 2 recent photographs in the size of 3.5 cms x 4.5 cms with white/light background.

6. Covering letter IN ORIGINAL signed by the applicant. For self-employed/businessman, going on tourist visa,
covering letter preferably on company's letterhead is required (We will provide the format)

7. CONFIRMED prepaid hotel reservation OR Invitation letter confirmation the accommodation provided by
Invitee.

8. Copy of income tax papers of last three year. If self employed in PVT Ltd or Partnership business 3 Year
Business Income Tax Paper required from Sponsor

9. 06-month bank statement & updated with bank stamp and sufficient balance from Sponsor.

10. Sponsorship letter from Parents with their Passport copy

11. Overseas Medical Insurance

12. Return Ticket

13. For Minor if traveling alone or with one parent required NOC from parent with their signature proof on
Notarized. (If parent is divorced then required divorce decree/court order for the same. In case of death than
required death certificate.

14. School/College ID for Students

15. Letter from College/Institute for Internship Program

16. All children within the age of 18years should provide a copy of birth certificate as a mandatory document.

17. Fixed deposit/PPF/PF or any other saving or investment documents (Optional) from Sponsor

Note: This is not the final checklist. There may be other documents required basis on the
documents of applicant.

Important!

 Self-employed applicants to submit proof of business i.e. firm's registration or partnership deed.
 Unemployed applicants and students can submit their spouse's / parent's financial papers AND a
letter of employment or business.
 All faxes must be photocopied and in A4 size paper.
 Copy of marriage certificate wherever necessary for e.g. if wife's passport is in maiden name.
 Each application must be supported by a copy of the requisite documents. If you have received a fax
you must photocopy the same and attach it to your application.
