1. Purpose of Data Collection


The PNP RSS may use collected personal information for the following purposes:

• To authenticate and verify identity;


• To process and assess requests or applications;
• To communicate with Data Subjects;
• To provide relevant information or updates;
• To improve our services;
• To comply with legal requirements; and
• Other legitimate purpose.

2. Types of Personal Information Collected


The PNP RSS collects personal identification information from individuals in
various ways, including but not limited to, visiting PNP Offices, engagement in PNP
services, completion of application/registration forms, or involvement in programs or
activities. The following personal information may be collected:
• Full name
• Address
• Contact number
• Email address
• Identification documents (e.g., passport, driver’s license)
• Biometric data (e.g., fingerprints, facial recognition)
• Other relevant details

By signing this form, l (as 'Data Subject") grant my free, voluntary, and
unconditional consent to the collection, use, and processing of my/our personal
information for the purposes stated above.

4. Rights of the Data Subject


The PNP acknowledges and respects the privacy rights of data subjects.
Individuals have the following rights:

• Right to be Informed whether personal data shall be, are being, or have been
collected or processed;
• Right to Object to the processing of personal data where such processing is based
on consent or legitimate interest;
• Right to Access personal data;
• Right to Rectification dispute the inaccuracy or error in the personal data;
• Right to Erasure or Blocking, suspend, withdraw, or order the blocking, removal,
or destruction of personal data;
• Right to Damages to be indemnified for any damages sustained due to such
inaccurate, incomplete, outdated, false, unlawfully obtained, or unauthorized use
of personal data;
• Right to Data Portability to obtain personal data in an electronic or structured
format; and
• Right to File a Complaint before the NPC if any data privacy rights have been
violated.

5. Disclosure of Personal Information


The PNP may disclose my personal information to authorized third parties for the
purposes of law enforcement and national security, as required by law.

6. Retention of Personal Information


The PNP RSS will retain my personal information for as long as necessary to fulfill
the purposes for which it was collected, unless a longer retention period is required by
law.

7. Data Protection
The PNP RSS shall implement reasonable and appropriate organizational,
physical, and technical security measures for the protection of personal information which
we collected.

Only authorized personnel are permitted and have access to the collected
information who will treat any confidential information under strict confidentiality. In case
of breach, the PNP shall notify you and inform the National Privacy Commission (NPC)
in accordance to the NPC Circular 1603 or Personal Data Breach Management.
