b. Memorandum from TDICTM/Data Privacy Officer dated October 8, 2024,
with the subject Implementation of Data Privacy Form.

2. This pertains to the importance of the use of the Data Privacy Consent Form
wherein the PNP is dedicated to safeguarding the privacy of individuals whose
personal information is collected and processed to ensure transparency and in
compliance with the Data Privacy Act of 2012.

3. The Data Privacy Consent Form serves to inform individuals about why their
personal data is being collected and seeks explicit consent for the use and
processing of their personal data, ensuring that individuals are aware of their rights
under the Data Privacy Act, including their
