PRIVACY NOTICE AND CONSENT

REGARDING PERSONAL INFORMATION ACCESS, TRANSFER AND STORAGE

(A) I have read and understood the Data Privacy Principle in the 3M Code of Conduct
and the Privacy Policy for 3M Service Center APAC, Inc. (“3M GSC-PH”);

(B) I hereby understand that, in my employment with 3M GSC-PH, certain personal and
administrative information will be collected, processed, or stored by 3M GSC-PH or
any of its affiliates (either in the Philippines, in the United States, or elsewhere),
and/or through 3M GSC-PH’s selected service providers (“Service Providers”).

(1) Personal information refers to any information whether recorded in material form
or not, from which the identity of an individual is apparent or can be reasonably
and directly ascertained or when put together with other information would
directly and certainly identify an individual, which includes, but not limited to,
information collected, processed, or stored by 3M GSC-PH and/or the Service
Providers including my name, home contact information, e-mail address,
telephone number, birth date, social security number, tax identification number,
passport number, driver’s license number, family background, educational
background, marital status, work experience, annual health examination results,
medical and criminal records, biological identification information (including, but
not limited to, fingerprints or facial images) and other information reasonably
related to my employment with 3M GSC-PH from which my identity can be
discernible (collectively, “Personal Information”).

(2) This Personal Information will only be used by 3M GSC-PH for identification,
security and personnel management purposes, such as, but not limited to,
providing building access, electronic systems access, payroll and compensation
processing, tax filings, managing employee benefits such as social insurances,
other group insurances and retirement plans, workforce monitoring, workforce
development and assignment administration, conducting background checks,
investigations and employee evaluations, M & A transaction, restructuring, and
other purposes and proceedings related to my employment with 3M GSC-PH
and permitted under the law (collectively, “Purposes”).

(3) I understand that 3M GSC-PH may use and provide affiliated companies and
unaffiliated third parties, located in the Philippines or overseas, including
governmental agencies, agencies which administrate salary payments, social
insurances, group insurances, retirement plans and other employee benefits,
external consultants, advisers, accountants/auditors, and other agencies and
service providers with my Personal Information, for the Purposes.

(4) I understand that the use of my Personal Information includes copying,

processing, analyzing, distributing, accessing, filing, displaying, printing, storing,
destroying, transmitting, and transferring of Personal Information.

 (5) At any time, I can request: (a) a list of the 3M GSC-PH affiliates or Service
Providers that have access to or store my Personal Information from the Human
Resources Department; or (b) a document detailing all the personal information
on me from 3M GSC-PH, its affiliates, or service providers.

(C) I understand that 3M GSC-PH will ensure that the Personal Information collected is
accurate, complete, and up to date, and that reasonable steps are taken to secure
stored or transmitted information, consistent with applicable privacy laws,
regulations and 3M GSC-PH Policies. In addition, 3M GSC-PH will use commercially
reasonable technical, physical and personnel measures to safeguard and secure my
Personal Information.

(D) In compliance with applicable privacy laws and in accordance with 3M GSC-PH
Policies, I am entitled to access, modify, update, and cancel my personal data, as
well as, to suspend/withdraw/or order blocking, removal or destruction thereof
upon discovery and substantial proof that my Personal Information are incomplete,
outdated, false, unlawfully used and/or obtained.

(E) When my Personal Information is collected, processed, or stored by Service

Providers, 3M GSC-PH will ensure that they maintain appropriate measures to
ensure the safekeeping and use of my Personal Information to cover exclusively the
purposes for which the Service Providers were hired.

(F) In my employment with 3M GSC-PH, I acknowledge that general Personal

Information will be available to 3M GSC-PH employees and other contract and
temporary employees to help facilitate my employment. These third persons are
required to use my Personal Information shared or disclosed with them only to
perform services on 3M GSC-PH’s behalf, to not use my Personal Information for
any other purpose, including that of said third persons, and to treat my Personal
Information as strictly confidential.

(G) 3M GSC-PH will retain my Personal Information during the entire duration of my
employment and five (5) years after my separation.

I hereby acknowledge that I have understood and I consent to the collection, use, and disclosure
of my personal and administrative information, including such activities that may have occurred
prior to my signing hereof, pursuant to the terms of this Privacy Notice and Consent letter.

I further acknowledge that any Personal Information I may have access to, whether intentionally
or unintentionally, in the course of my work in 3M GSC-PH shall be treated by me in the same
manner and with the same diligence and care as outlined above.
