TRAINING CONFIRMATION AND AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Training Confirmation and Agreement, made and entered into this ______ day of _________________ 2024 in
______________________________________________________, Philippines, by and
between:_________________________________________________________________, a corporation duly organized and existing under
Philippines laws and with office address at the Suite 307 Grace Bldg. Ortigas Ave. Greenhills San Juan, City Philippines, represented herein by
its representative, ______________________________, hereinafter referred to as the COMPANY;

-AND-

___________________________________, a Filipino, of legal age, single/married, and a resident of


___________________________________________________________________ __________________________________, hereinafter
referred to as the EMPLOYEE.

WITNESSETH THAT:

WHEREAS in consideration of the premises the Employee has agreed to enter into this Agreement upon the terms and
conditions hereinafter appearing.

1. The Employee voluntarily agrees to attend and complete the Training / Seminar / Workshop for
___________________________________________ as required by the position to enhance the skills.

2. The Company shall pay the Expenses for the said Training amounting to Php ________________.

3. Upon successful completion of the Training, the Employee undertakes to serve the Company for a minimum period
of one (1) year, (hereinafter referred to as "the Contract Period"). The Contract Period commences from the date of
the Employee's employment with the Company after returning from the Training.

4. The Company has the right to revoke or terminate this Agreement if at any time during the Training the Employee
has committed any of the following:

a) Misconduct;
b) Performance related concerns
c) Abandonment of the Training before the completion thereof;
d) Any breach by the Employee of any of the provisions of this Agreement.
e) Other related issues according to the General Policies and Procedures or GAPP and
Labor Code of the Philippines.

5. If the Employee shall voluntarily resign from the Company before the expiration of the Contract Period, the number of
liquidated damages shall be calculated on a proportionate basis.

6. Any payment which the Employee is liable to pay to the Company under this Agreement shall be deducted from the
final pay claims.

7. I hereby authorized the Company, WERT PHILIPPINES INC. to deduct from my last salary or the remaining costs
incurred from any money due me and/or pay, the cost of said training/seminar/workshop.

8. That I’ll echo the input learned from the training/seminar/workshop to fellow or team workers.

9. All training manuals if any, MUST be surrendered to the Human Resource Department together with the clearance
form duly acknowledge by the Immediate Superior.
