PROBITIONARY CONTRACT

Dear Mr./Ms._________:

We are pleased to inform you that your employment to our company is (date hired) as
(position).

1. You will be on Probationary for (6) months from ______________ to (5th month),
during which period you may be laid off at any time for cause, or for failure to perform
your duties and responsibilities in accordance with standards prescribed by the
company.

2. Working Schedule: Monday to Saturday – 8:00am to 5:00pm

3. Your compensation are as follows:

4. Upon completion of your probationary employment, your employment is continue for


regularization if your performance evaluation resulted to satisfactory, otherwise, your
services will be terminated, upon transfer to permanent status, you will be entitled to
a permanency increase which will be based on performance evaluation rendered by
your immediate superior.

5. You will be entitled to company benefits from the date of your permanent
appointment.

a) Leave Credits – (3) Three Sick Leave and (3) Three Vacation Leave

6. Upon signing of this Agreement, it is expected that you shall conform, follow and
abide by the established Company Rules and Policies and faithfully observe all
pertinent duties and corresponding responsibilities as employee of the company.

7. Outside Business Activities – During your employment with Company, you shall
devote competent energies, interests, and abilities to the performance of your duties
under this Agreement. During the term of this Agreement, you shall not, without

Company’s prior written consent, render any services to others for compensation or
engage or participate actively or passively, in any other business activities that would
interfere with the performance of your duties hereunder or compete with Company’s
business.

8. Withholdings – All compensation paid to you under this Agreement, including


payment of salary and taxable benefits shall be subject to such withholdings as may
be required by law or Company’s general practices.

9. Upon signing of this Agreement, it is expected that you shall conform, follow and
abide by the established Company Rules and Policies and faithfully observe all
pertinent duties and corresponding responsibilities as employee of the company.

Should the foregoing conditions are acceptable to you; please indicate your
conformity by signing on the space below.
