1. Introduction
i. Mission Statement
The goal of FIRST ARAB is to achieve Incident Free and Injury Free workplace. The
management including our entire workforce will take all measures to achieve this target, complying
with all SIPCHEM - HSE requirements, standards and specifications is our sincere desire to
complete this project without sustaining anyinjuries and damage to property / environment, and we
shall expect each support in so doing. Everyone commitment to this HSE plan, their participation
and cooperation are essential to meet our project and Project objectives. The project safety culture
can be manifested in this statement:
"The work is never so urgent that we cannot take the time to do it safely. It is the
responsibility of each individual that everyone leaves the job in the same condition they
arrived."
Everyone is accountable for the attainment of this target towards our safety excellence and
management prime responsibility is to ensure success of this commitment.
ii. Project HSE Philosophy
FIRST ARAB Philosophy is a commitment, and to achieve our safety excellence everyonemust
accept the following:
➢ All accidents are preventable.
➢ No job is so important, and no service is urgent that we cannot take time to do it
safely.
Only Incident Free and Injury Free Workplace is expected to be consistent with goodproductivity
and profitability.
2. Purpose
The purpose of this document is to define the basis for the construction of Car Parking Shed project
includes civil works, MEP works at Jubail.
Its primary purpose is to provide guidelines for preventing any accidents which may result in injury
to employees and damage to property / environment. The implementationof this HSE Plan will
create a proactive approach to any circumstances that may possibly develop / convert into an
incident / accident, injury, and damage, regardless of severity during the Project execution.
3. Scope
This HSE plans applies to all First Arab employees and its Subcontractor(s) during the execution
of this project.
This HSE Plan defines the standard that is required to be adopted by First Arab which makes
references to First Arab own documented management system and aligned with SIPCHEM - HSE
requirements, standards and specification forworking safely during the project execution.
In case of conflicting HSE requirements, standards and specifications between the Company and
the Contractor, the most stringent reference / document must prevail.
4. Policy
The management of First Arab Company Limited has full committed to the goal of providing and
maintaining a healthy and safe working environment, with a view to continuous improvement. This
goal is only achievable by adherence to established objectives striving to exceed all obligations
under applicable legislation, and by fostering an enthusiastic commitment to health, safety and the
environment within First Arab personnel, contractors and visitors.
FIRST ARAB COMPANY recognizes that the responsibility of employee safety and health requires constant and
continuing joint efforts of Company Management, its Employees, Sub-Contractors and Client.
We shall comply with all legal and regulatory requirements that are implemented in the Kingdom of Saudi Arabia and
generally accepted work practices and procedures for the protection and promotion of the safety and health of its
employees and others, who may be affected by the Company’s activities, and shall strictly adhere to all site Safety,
Health & Environment standards, work rules and practices.
Employees at all levels shall work conscientiously and diligently to execute the FIRST ARAB HSE policy of maintaining
the highest standards of occupational health and safety to prevent human suffering and losses, which result from
unsafe practices and lack of protection of the Environment.
FIRST ARAB COMPANY shall promote and enforce the development, dissemination, implementation, effectiveness
and continual improvement of its HSE program. For this purpose, the Management shall commit adequate resources
and finance.
FIRST ARAB COMPANY Management empowers its employees to stop their own work and the work of others
whenever they find any unsafe conditions or acts.
This policy will be reviewed yearly or earlier if required. It will be amended to keep it in line with current legislation,
codes of practice and to remain appropriate with our activities.

Project Management by the conjoint lead of Project Manager and HSE Officer shall commit to
perform construction works under the scope of this Project in a manner that is uncompromising on
issues of health, safety and environment.
and any other person involved in Construction and preventing damage to the environment, existing
facilities and surroundings.
The PROJECT goal is to have “NO INJURIES” (Zero LTI) and everyone must do their utmost to
reach this goal;
Everyone in the Project shall strive to promote and achieve a fully committed Safety partnership
with all parties involved on the PROJECT.
SIPCHEM Administrator(s) shall provide First Arab with a copy of the ESH Manuals, which outlines
and specifies safety requirements needed to be adhered to.
First Arab shall be responsible for the safety of their employees and shall comply with the Project
safety standards, requirements, and procedures.
Accountability is a critical part of the safety program and will be implemented with positive
reinforcement that will challenge everyone involved, irrespective of his level of responsibility, to
achieve our mutual goal of Incident Free and Injury Free Workplace.
All activities will be governed by the perception that, all accidents can be prevented and that the
behavior of all employees can be modified to work safely. In order to achieve an Incident Free and
Injury Free culture, all levels of management are required to subscribe to the following:
7.1. Management
To ensure the proper implementation of HSE plans and procedures, First Arab has a disciplined
management structure. This structure helps to promote cooperation between individuals, HSE
representatives and top management so that HSE becomes a collaborative effort. This structure
also ensures dissemination of necessary information throughout the project at all levels.
The First Arab Management has an indirect role at the project by ensuring that the Health, Safety,
and Environmental objectives of the project are adhered accordingly, and this will be achieved by:
• Fully enforcing adherence of the agreed Project HSE Plan & other HSE deliverables.
• Selection of experienced and competent employees. Thus, organizing a Project
management team committed to the Project’s HSE plan and objectives.
• Detailed and appropriate planning of site activities, to coordinate/ minimize/ controlcontractor
interfaces with required number of people on site.
• Employing correct controls for effective management of safety, and to address anyidentified
risks.
• Introduction of an effective communication system.
• Monitoring health, safety, and environmental performance.
• Keeping up to date with the project health, safety and environmental matters / issues and
continually improving HSE Management System
• Regular review of the HSE management system and updating of the Construction HSE Plan to
facilitate constant improvement of the management system as per the Safety strategy for the
Project.
• Establishment of awareness and introduction of incentive programs.
• Proving involvement and showing the expected HSE commitment and leadership.
• Take reasonable steps to protect their self and to protect the health and safety of others through
their responsible supervision.
• Provide full assistance to the Project Manager for implementing, maintaining, documenting the
HSE management system and complying with the Client’s or Project’s requirements.
• Aid in determining resources needed for the establishment, implementation, maintenance, and
continual improvement of the HSE Management system.
• Exercise their HSE function, rights and authorities undertaken with good interpersonal skills.
• Conduct regular workplace surveillance to determine potential OH&S risks andenvironmental
impacts.
7.5. Craft worker
Craft workers, like any other person on the project, have the responsibility to keep himself and the
people around him safe. They work under the supervision of their corresponding line supervision
and have the responsibility to perform their work safely according to the training they received and
instructions from their superiors. Craft workers have the responsibility to ask when they are not sure
and must think before they act. They are also responsible to coach co-workers should they not
understand the work and/or safety requirements and therefore are about to perform an unsafe act
or create an unsafe condition. They shall:
• Take reasonable steps to protect their self and to protect the health and safety of others who
might be affected during the business operations.
• Understand, be familiarized and comply with the HSE management system requirements and
its processes.
• Participate by exercising their right: to enquire about OH&S risks, environmental impacts and
preventive measures; and to remove themselves from dangerous situations and to consult,
whenever appropriate, for continual improvement.

• Where appropriate, provide appropriate notification and / or participate in the conduct of incident
investigation of all work-related incidents related to Health, Safety and Environment.

8. HSE communication
First Arab shall abide by all safety rules and other regulations imposed at the site by the laws of
country and comply with all applicable Code of Practice, local, International Standards, regulations
including rules and procedure as applicable from the Project.
All employees will attend site-specific orientation training, presented at work site prior to the start
of work, or as required when site conditions change. This might be when there is a change in the
construction phase. The goals set for the project, the project safety rules and regulations and the
No Accident philosophy will be communicated to all employees, supervisors, and managers. The
course will also emphasize the importance of human life and promote employee ownership and
accountability by utilizing behavior-based safety techniques.
First Arab shall submit its employees, as applicable, to attend the following SIPCHEM’s HSE
Training plan
- All new employees shall undergo the Site HSE Orientation prior to mobilizing them for work at
the project. New employees should understand HSE Policy and shall adhere to all HSE
regulations implemented at project site.

- All First Arab’s employees will be provided with PPE at free of cost
- First Arab’s site management shall provide Personal Protective Equipment (PPE) to all levels
of employees that are in accordance with SIPCHEM’s PPE standards; following are the basic
items of PPE that shall be provided to workers and shall be worn while working.
✓ Safety Shoes
✓ Hard hat
✓ Gloves, Safety glass / goggles
✓ Others as may be required of the Task
- All work activities shall be performed under a valid permit to work and under the close
supervision.
- Prior to start of work, Work Permit may be required and must be at place of work. Work permit
shall be obtained prior to start of any activity. No work will be carried out without the work permit
and TBT meeting shall be held with all involved employees.
- Workers shall be provided with adequate supply of potable drinking water particularly in summer
months.
- All incidents shall be reported immediately by any employee assigned at site. Incident
notification flow shall be followed for effective communication and quick response of responsible
personnel.
- First Arab Site Supervisors shall not allow any worker to work alone in a confined space; they
must follow buddy system and all activities in a confined space must comply with the Confined
Space requirements. All personnel involved in confined space operations shall have attended
the mandatory confined space entry training.
- All employees must be aware of Site emergency procedures. They must know locations of
assembly area, escape routes, emergency contact numbers etc. Employees shall follow project
site emergency response procedure which shall be Discussed / explained in the Safety
Orientation.
- Work at height is strictly prohibited without appropriate fall arrest systems; scaffoldings shall be
provided with railings; no gaps; all planks must be secured appropriately. Scaffold tag system
shall be observed; employees shall not carry tools or materials while climbing up or down
scaffolding ladders.
- Smoking shall only be allowed in designated Smoking Area and cigarette butts should be
discarded in a proper receptacle.
- Vehicle drivers shall observe speed limits while driving at construction and site facilities; drivers
must have a valid driving license for the class of vehicle he is operating or driving. Vehicles must

be parked in designated parking areas; vehicles must be equipped with seat belt.
A TBM is one of the most effective means of communication, in eliminating accidents and
enhancing the safety awareness of all site personnel. The TBM is a short meeting by the individual
work groups, and is held in the close vicinity of the work location, prior to starting the work.
The group leader or Supervisor who responsible for the job and permit applicant shall takes the
initiative for conducting the TBM on a regular basis.
All the work crews including Subcontractor’s supervisors, foremen and workers shall actively
participate in the TBM.
The working plan and procedures for the day are explained and confirmed including the following:
8.4. The record of weekly TBM shall be attached together with Work Permit place in the permit
box that is conspicuously displayed at the site. A record of each meeting topic and meeting
attendance shall be kept and compiled on a daily basis for future references.
This meeting will be conjointly attended by HSE personnel and construction key Personnel or
Representatives. The HSE Meeting shall be held weekly or as required and leaded by the HSE
Officer:
• Site Management will lead the weekly site walk through in all project facilities.
• Safety Inspection will be documented and records will include dates of the inspection, a
copy of the checklist for items inspected, and action taken to correct violation and follow-up
reports.
• First Arab Safety Officer jointly with Site Engineer/Supervisor will conduct daily inspection,
and documented the same in safety file system.
• The findings during inspection will be rectified immediately and follow up will be done by
safety personal to avoid further repeated actions.
• After the site inspection, the First Arab team will provide the detailed reports, where unsafe
conditions, practices are noted to immediate rectification.
• First Arab site management will conduct an internal meeting to discuss all the concerned
site deviations.
• The concerned department will rectify the observed site deviation immediately as per the
given time frame written on the reports.
• Audit team will revisit the area to ensure unsafe conditions, practices has been rectified as
per the given time frame.
• Failure of compliance towards the comments, the concerned team shall immediately stop
the work until they correct the comments.
The meeting shall be chaired by Project Manager shall organize upon after any incident, high
potential near miss, dangerous occurrence and emergency situation, and security incident, etc.:
Additional meetings may be called by SIPCHEM HSE Management and required to be attended by
First Arab key personnel to address special circumstances such as:
To ensure that all site personnel working at Project are provided with appropriate Personal
Protective Equipment (PPE) to wear and protect them against work related hazards which may
endanger their health and safety.
9.1.2. Objective
PPE shall not cause an unacceptable encumbrance to the wearer, shall fit the wearer correctly and
shall be used to reduce risk of harm to an individual. It should be noted that PPE may reduce the
risk of harm or injury occurring to an individual but should only be used as a means of last resort.
9.1.3. Instructions
The risks posed in any particular work activity shall be assessed, and adequate PPE selected in
accordance with the following criteria:
a) Must be appropriate for the risks involved and conditions of the workplace where exposure to
risk may occur;
b) Must have good ergonomic design and fitted to the wearer (worker).
c) Must be compatible with other items of PPE that may be required to be worn at the same time.
d) Must comply with minimum standards approved by SIPCHEM HSE Management.
• All employees shall be provided with necessary PPE required for work activity, they shall also be
provided with instruction and training for correct usage of PPE, if required – training courses
through Mass Tool Box Talk shall be provided to all employees at site.
• All employees shall be held responsible for the proper care and use of PPE. They shall wear
PPE at all time while at project site.
• Site supervisors shall impose disciplinary action to any employee who violated regulations of this
procedure.
• First Arab shall replace, free of charge any PPE which becomes deficient through normal wear
and tear.
b. Safety footwear with steel toe & steel sole protection must be manufactured in accordance with
the standard ANSI Z41.1-1967
c. Safety glass shall be used with side shield and in accordance with the standard ANSI Z87.1-
1968
d. Respiratory 3M 6006 is mandatory.
e. Notices displayed stating that a particular PPE shall be worn. Visual signs shall also be
displayed.
Hearing protection shall be provided to all workers exposed to high noise levels (exceeds 85 DBA
TWA) at workplace as per international standards. Selection of hearing protection shall be in
accordance with the type of noise hazard and the work being performed.
9.1.4.6 Harnesses
Harness shall be provided, worn and properly secured while working at heights greater than 1.8
meter. Such situations include, but are not limited to:
9.2.1 General
This procedure describes a three-step, systematic approach to be used by First Arab to protect its
employees on the project from falls when they are working at elevated positions or are exposed to
potential fall hazards. This procedure explains how to eliminate fall hazards, prevent falls, and
eliminate or reduce injury if a fall does occur.
9.2.2 Definitions
Anchor Point - A secure point of attachment for lifelines, lanyards, or deceleration devices. An
anchorage must be capable of supporting a minimum dead weight of five thousand (5,000) pounds
(2,268 kilograms) for each person attached to it. An anchor point is often a beam, girder, column, or
floor.
Competent Person - Any personnel who has been trained to inspect fall-arresting equipment such
Lanyard - A rope (nylon or steel cable) suitable for supporting one person.
Lifeline - A vertically suspended rope with one end attached to a stationary object (such as a
structural member), capable of supporting at least five thousand (5000) pounds (2,268 kilograms) of
dead weight and the other end attached to a lanyard or safety harness.
Retractable Lifeline - A fall-arrest device that allows free travel, without slack rope, but locks
instantly when a fall begins. Retractable lifelines may be used, but horizontal movement must be
limited.
Rope Grabs (Fall Arrestor) - Automatic lifeline devices that act by inertia (resistance to movement)
to grab the lifeline if a fall occurs. Rope grabs are used when vertical movement is required, such as
work from boatswain chairs or suspended scaffolds.
Safety Harness - A safety harness is an approved design of straps which may be Secured about
the employee’s body in a manner to distribute the fall arrest forces over at least the thighs, pelvis,
waist, chest, and shoulders, with a means for attaching it to other components of a personal fall
arrest system.
Static Line or Catenary Line - A cable or rope strung horizontally and/or vertically from one
substantial object to another, providing a means of traveling between those two objects while
maintaining fall protection between those objects.
Structural/Substantial Object - Any object to which a lifeline or lanyard may be attached that will
support five thousand (5000) pounds (2,268 kilograms) of dead weight.
First Arab must take a three-step systematic approach to protecting employees from falls.
The first step in this approach is to assess the workplace and the work itself in the earliest
design/engineering stages of the project/site and during the planning stages of all work. The objective
is to eliminate all fall hazards. This assessment of the project/site and the work not only helps
eliminate hazards, but also identifies alternative approaches to the work that can measurably

enhance productivity.
Addressing fall protection in the early phases of a project/site means that safety can be designed
into the work process. For example, the project/site can be designed so that anchorages for securing
fall-arrest systems are provided at strategic locations throughout the project/site, thus improving
safety and lowering costs.
The second step in continuous fall protection also requires assessing the workplace and work
processes. If fall hazards cannot be completely eliminated during the first step, management must
take a proactive approach to the prevention of falls by improving the workplace. Early installation of
stairs, guardrails, barriers, and travel restriction systems can ensure a safe work environment.
The third step, the last line of defense against falls, is to use fall-arresting equipment. Use fall-
arresting equipment, however, ONLY after determining that potential falls cannot be eliminated by
changing work procedures or the workplace. Equipment such as harnesses, lanyards, shock
absorbers, fall arresters, lifelines, anchorages, and safety nets can reduce the risk of injury if a fall
occurs. Carefully assess the workplace and work processes to select the most appropriate
equipment and to install and use it correctly.
9.2.3.4 Training
All personnel performing work at elevated positions shall meet the following requirements:
- must be trained in the use, inspection, and maintenance of fall arrest systems;
- must demonstrate competency in initial training;
- Must undergo refresher training as required by changes in the workplace and/or equipment, or as
other events indicate that refresher training may be required.
When working at an elevation of 1.8 meters (six feet) or more aboveground, grade, floor, or approved
work surfaces such as platforms and scaffolds, or when working in an area where a fall potential of
greater than 1.8 meters exists, employees will utilize a full body harness with two lanyards and shock
absorbing unit and proper means of attachment.
If traversing at an elevation, employees must be tied off at all times, which require utilizing either two
(2) independent shock-absorbing lanyards or a dual (Y) lanyard with a shock-absorbing device.

9.3. Housekeeping
9.3.1 Purpose
To outline the requirements for managing and controlling housekeeping operation during the project
execution.
9.3.2 Scope
This shall apply to all construction activities and site facilities (office, workshop, and site) project site.
9.3.3 Definitions
• Housekeeping is an act of keeping the workplace cleared of all unnecessary waste and materials.
• Housekeeping is a primary responsibility of all personnel; the line management’s commitment
shall be demonstrated by a continued effort of the supervisory staff towards this activity.
Project In-Charge
a) Designate responsible person to handle control of waste materials; to monitor site conditions,
general housekeeping, wastes disposal etc.
b) Assign an approved waste removal company to collect/remove waste materials at site; ensure
that waste material is reaching the landfill area or incineration facility, and is not being “flying
tipped” or causing environmental damage.
9.3.5 Instructions
These instructions shall apply to site offices, workshops, and project site during the course of the
project.
a) All emergency exits, passageways, exits, fire doors, break-glass alarm points, firefighting
equipment, first aid stations, and other emergency stations shall be kept clear, unobstructed and
in good working order.
b) All scaffold materials shall be stored and arranged in orderly manner at all times.
c) All loose scaffold materials stored at the elevated workplace must be secured and not placed
nearby leading edges.
d) All workplaces, accesses and means of escape shall be maintained clear of debris, waste and
other rubbish, which shall be disposed-off in segregated containers for disposal.
e) All liquid spillage (oil and grease) shall be immediately cleared using a spill kit.
• Crane Lifting plan shall be prepared and submit for review from the client. Work shall be
performed after getting approval of Lift plan from the SIPCHEM.
• Lift Plan shall be prepared by the competent person / lifting Supervisor.
• Crane (Equipment), Operators, Signaler and ancillaries should have valid certificate and Trail Lift
Shall be performed by holding a load for 3 seconds from the ground.
• Crane’s SWL should be displayed on it and crane should be well maintained, in good condition
to perform the lift.
10.1 General
First Arab will comply with the manufacturer’s specifications and limitations applicable to the
operation of any and all cranes and derricks. Where manufacturer’s specifications are not available,
the limitations assigned to the equipment shall be based on the determinations of a qualified engineer
competent in this field and such determinations will be appropriately documented and recorded.
Attachments used with cranes shall not exceed the capacity, rating or scope recommended by the
manufacturer.
• Rated load capacities and recommended operating speeds, special hazards warnings or
instructions shall be conspicuously posted on all equipment. Instructions or warning shall be
visible to the operator while he is at his control station.
• A test lift using the equivalent outrigger loads shall be undertaken for all lifts over 20 Tons.
• Lifting of loads over 10ton will be having a standard lift plan with the crane radius and boom
length along with load chart calculated manually to verify along with the load monitor installed
on crane and with all ancillaries having 3rd party certificates.
• All lifting of loads of 50ton will have a critical lift plan prepared and approved by
• relevant personnel, before starting the work.
• Hand signals to crane and derrick operators shall be those prescribed by the applicable
standards for the type of crane in use. An illustration of the signals shall be posted at the crane
for quick referencing.
• First Arab will designate a competent person who shall inspect all Machinery and equipment
prior to each use and during use to make sure it is in safe operating condition.
• Any deficiencies shall be repaired or defective parts replaced, before continued use.
• The following wire rope inspections will be performed in accordance with manufacturer’s
recommended time frame.
• In running ropes, six randomly distributed broken wires in one lay or three broken
• wires in one strand in one lay.
• Wire rope safety factors shall be in accordance with American National Standards Institute
B30.5-1968 or SAE-959-1966.
• Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheel, chains or other reciprocating,
rotating, or other moving parts or equipment shall be guarded if such parts are exposed to
contact by employees, or otherwise create a hazard, guarding shall meet the requirements of
the American National Standards Institute B15.1- 1958 Rev. Safety Code for Mechanical Power
Transmission Apparatus.
• Accessible areas within the swing radius of the rear of the rotating superstructure of the crane,
either permanently or temporarily mounted, shall be barricaded in such a manner as to prevent
an employee from being struck or crushed by the crane.
• All exhaust pipes shall be guarded or insulated in areas where contact by employees is possible
in the performance of normal duties.
• Whenever internal combustion engine powered equipment exhausts in enclosed spaces, tests
shall be made and recorded to see that employees are not exposed to unsafe concentrations of
toxic gases or oxygen deficient atmospheres.
• All windows in cabs shall be of safety glass or equivalent that creates no visible distortion that
will interfere with the safe operation of the SIPCHEM Machine.
• Where necessary, for rigging or service requirements, a ladder or steps shall be provided to give
access to a cab roof.
• Guardrails, handholds and steps shall be provided on cranes for easy access to the car and cab,
conforming to ANSI B30.5
• Platforms and walkways shall have anti-skid surfaces.

allow spill or overflow to run onto the engine exhaust or electrical equipment of any Machine

➢ No cranes shall be re-rated in excess of the original load ratings unless such rating
changes are approved by the crane manufacturer of final assembler.
• Handling the load: size of load, no crane shall be loaded beyond the rated load, except for test
purposes. Overloading during testing will only be permitted by, or with the full consent of, the
manufacturer.
• When loads, which are limited be structural competence rather than the stability are to be
handled, it shall be ascertained that the weight of the loads has been determined within plus or
minus 10 % before it is lifted.
• Attaching the load: No part of the crane’s rope shall be wrapped around load. The load shall be
attached to the hook by means of slings or other approved devices.
• When moving the load, the employees shall assure the load is well secured and properly
balanced in the sling or lifting devices before it is lifted more than five inches.

• Before travelling a crane with load, a designated person shall be responsible for determining,
and controlling safety. Decisions such as position of load, boom Location ground support, travel
route, and speed of movement shall be in accord with his determinations.
• Crane or without load shall not be travelled with the boom so high that it may bounce back over
the cab.
• When rotating the crane sudden starts and stops shall be avoided. Rotational speed shall be
such that the load does not swing out beyond the radii at which it can be controlled. A tag or
restraint line shall be used when rotation of the load is hazardous.
• When a crane is to be operated at a fixed radius, the boom- hoist pawl or other positive locking
device shall be engaged.
• Ropes shall not be handled on a winch head without the knowledge of the operator.
• While a winch head is being used, the operator shall be within convenient reach of the power
unit control lever.
10.1.6 Cabs
• Necessary clothing and personal belongings shall be stored in such a manner as to not
interfere with access or operation.
• Tools, oil cans, waste, extra fuses, and other necessary articles shall be stored in the toolbox
and shall not be permitted to lie loose in or about the cab.
10.2.1 Sling
Safe working loads (expressed in short ton of 2,000 lbs.) will be marked on all shackles and these
safe working practices will be followed:

10.2.3 Hooks

Traffic Regulations that every driver / crane operator must be aware of:
a) Driver's license
Motor vehicle drivers and equipment operators shall be qualified and licensed in accordance with
the regulations of Saudi Law. Vehicle drivers will be in possession of a valid Saudi driver’s license
and equipment operators will be in possession of a valid Saudi equipment operator license specific
to the type of equipment operated and a competency certificate issued by management.
All vehicles on the property will be there at the risk of the vehicle’s owner. Neither Client / nor Project
Management Team accepts responsibility for damage to or theft of such vehicles.
g) Right-of-way
All motor vehicles shall give right-of-way to pedestrians, ambulances, and firefighting equipment and
other emergency vehicles. All drivers of motor vehicles shall practice extra ordinary driving courtesy.
h) Passengers
Passengers shall be limited to the number according to the design of the vehicle and manufacturer’s
specification and the vehicle shall not be overcrowded. Drivers of motor vehicles shall not permit
passengers to ride on fenders, running boards, tops, or bumpers of motor vehicles and in the back
of pick-up trucks. Passengers shall keep all parts of their bodies inside the cab or body of vehicles
and must be seated while vehicle is in motion. Tailgates must be kept closed. Passengers shall not
get on or off a vehicle while it is in motion.
i) Windshield or window obstruction
Vehicles having nontransparent window materials, which interfere with clear visibility through any
side window or windshields with cracks in the windshield shall not be operated on the project. All
vehicles shall be equipped with adequate rear-view mirrors.
j) Back-up alarm
All heavy vehicles and motorized equipment shall be equipped with a back-up alarm that is audible.
First Arab shall designate a person who is trained and experience in hand signals used and
understood by crane operators.
• One designated signalman will be responsible for the control and coordination of any particular
lift or Heavy Rigging Activity and shall wear a green reflective vest.
• Under certain conditions, such as multiple crane lifts, or lifting "in the blind", it may be deemed
necessary for additional signalmen to participate in a lift or Heavy Rigging Activity. One
signalman will be designated as the “Lead Signalman”.
• All designated subordinate signalmen shall be ultimately directed and coordinated in their efforts
by the designated lead signalman.
• All equipment Operators participating in any Heavy Rigging Activity shall be instructed to follow
the directions of the designated lead signalman, except for emergencies, and shall ignore all
other directions and signals.
10.3 Heavy mobile equipment
First Arab shall mobilize Heavy Equipment that is in good and serviceable condition and shall be
maintained on the entire project duration. The Heavy Mobile Equipment must be submitted to
SIPCHEM for a safety inspection and review of the operator’s license and competency certification,
insurance documents and third-party inspection certificate. First Arab shall only utilize Company and
Contractor’s approved third-party inspection companies.
Small repairs shall be done on site with provision of spill kit and drip tray, Oil changing of First Arab
vehicles shall be outside of project premises and used oil shall be kept 200L drums and shall be sent
for recycling at main branch Jubail with the quantity record.

The operator shall perform documented daily inspections before use on each shift. Flagman must
be used when maneuvering on constricted area or during backing operation at all times.
The project/site or any individual without the manufacturer’s written approval shall make no
modifications or alterations which could affect the capacity or safe operation of the equipment.
10.3.1 Operator training and licensing
All Heavy Mobile Equipment operators must review and understand the manufacturer's operations
manual for the assigned make and model MP Chine, applicable OSHA and ANSI standards, and any
additional applicable information concerning the operation of their assigned equipment.
They must be licensed By the Saudi government to operate the specific make and model of
equipment to which they are assigned.
Regular inspections and maintenance of the cranes shall be conducted and performed in accordance
with the manufacturer's specifications and American National Standards Institute (ANSI) standards
or other applicable in-country standards. Maintaining the crane in good working condition will
eliminate unnecessary downtime and to prevent possible accidents. Utilizing contractors must be
able to provide documentation concerning crane condition, which should be monitored regularly.
• The minimum illumination for all night work will be 10 feet candle (10 lux). Sufficient warning lights
must also be allocated in specific areas to warn other Contractors and traffic of the works being
carried out in the area. These warning lights are supplied and maintained by the Contractor.
10.4 Traffic sign
All SIPCHEM project road, Saudi Arabia Government Road and Traffic Signs shall be observed and
complied by First Arab Driver’s and Operators at all times.
Consequently, anyone caught not complying this instruction will be dealt accordingly with appropriate
Disciplinary Sanctions.
Appropriate and applicable safety signage and banners must be provided at the workplace to
intensify information drive and to make some alerts. Additionally, use of caution and warning tape /
and or physical shall be adapted during the scaffold construction execution.
• Any activity shall have an approved Permit (PTW) prior to start of work. Permit to work (PTW)
system will be adhered in the execution of this project. All permits shall be accompanied with
approved Risk Assessment and its corresponding Method Statement.
• A permit-to-work is a document which specifies the work to be done and the precautions to be
taken. Permit to work form an essential part of safe systems of work for many maintenance and
construction activities.
• The permit-to-work form must help in communicating hazards and precautions between everyone
involved. It should be designed by the client issuing the permit, considering individual site
conditions and requirements, so that sufficient emphasis can be given to the particular hazards
present and precautions required.
• It is mandatory for all permit receivers to attend a permit to work system training. Work permit is
required prior to start of work as per requirement and in accordance with permit to work system
procedure.
• Identifying Hazard Checklist and Risk Assessment must be attached on the permit and
conspicuously displayed at prominent locations.
It also provides guides and covers the organizational, individual roles and activities of personnel
tasked with specific responsibilities with regard to the establishment and maintenance of a Fire
Prevention/Protection and Fire evacuation Plan.
f) No welding activity shall be done on any part of the superstructure of a temporary office once it is
completed.
g) Temporary office buildings are to be positioned in such a way that it will ensure optimal
accessibility during emergencies. All temporary office buildings shall be properly grounded.
h) All Temporary office buildings shall be installed with smoke detectors and a means of alarming
system. Fire Marshal, with adequate training and experience in fire prevention and firefighting will
be appointed to monitor overall fire prevention and protection at the Temporary Facilities.
i) Escape routes and assembly area will be indicated in the Temporary Facilities.
j) Good housekeeping shall be enforced and maintained at all times in all areas.
k) Upon completing temporary office buildings, it shall be immediately equipped with the correct type
and number of fire extinguishers. It shall be placed strategically at office entrances, exits and
inside the building. It shall be positioned in such a way that the locations of fire extinguisher are
not more than 15 meters apart.
l) Space between elevated and ground of temporary office facilities be closed to prevent occurrence
of the following:
• Accumulation of vegetative growth and combustible materials underneath the buildings.
• It will avoid usage of the space as storage area.
10.8 Site transportation
All personnel performing work shall be transported to and from the construction site in proper buses
equipped with proper seating and emergency exits for personnel transportation.
10.8.1 Driver Requirements
Only qualified personnel shall drive & operate motor vehicles. Each driver shall possess and have
on his person a valid SAG driver’s license for the type of vehicle being driven.
10.8.2 Drivers Responsibility
Each driver shall ensure that the vehicle he is required to drive is safe to operate, is serviced, and
maintained to schedule and that all defects are rectified. The driver shall be responsible for the safe
transportation of any load in the vehicle, ensuring the vehicle is not overloaded and that the load is
secure. Drivers and passengers of all moving vehicles fitted with seat belts shall wear them. Drivers
shall not move the vehicle until all passengers are secured. Drivers shall not exceed the posted
speed limits and shall obey all traffic signs and regulations.
10.8.3 Rules on Transportation
No person is allowed to ride on the running board, step board, hood or fenders of vehicle. Only First
Arab personnel are allowed to ride on company vehicles. Police, ambulance, fire trucks and other

vehicle responding in an emergency shall be given the right of way. In case of accident, the driver
concerned shall not leave the scene of accident without aiding the victim, nor shall he move his
vehicle away from the scene unless its causing hazard or if he is going to get medical aid for the
victim. Passenger riding in a vehicle shall be properly seated inside the vehicle and they shall be
under the direction of the driver. Sitting on the sides or back of open pick up or trucks is prohibited.
No passengers or riders are allowed on bulldozers, loaders, graders or any other unit not designed
for transportation.
Passengers and unsecured materials should not be transported in the same compartment at the
same time. Vehicles should not be overloaded with passengers or material.
Heavy item such as tools or water cans must be secured or tied down. Materials must not extend
over the side of the pick-up trucks. When inevitable, a warning sign shall be provided to caution the
other traffic.
Loads extending beyond the rear of the vehicle must be marked with a red flag in the daytime and a
red light at night.
a) Lightning – All large-scale, high tension, natural, electrical discharge in the atmosphere; the
visible flash of light accompanying such discharge.
b) High Winds and Rain - When the winds or rain become a hazard to the work force or the work
being performed. Wind 20 mph for cranes operations and approximately 30 mph and rain are
limiting visibility or footing.
c) Dust and Sandstorm (DSS) – dust storm and sand storm is a meteorological phenomenon
common in arid and semi-arid regions. Dust storms arise when a gust front or other strong wind
blows loose sand and dirt from a dry surface. Particles are transported by saltation and
suspension, a process that moves soil from one place and deposits it in another.
d) Thunderstorm – is a type of storm characterized by the presence of lightning and its acoustic
effect on the Earth's atmosphere known as thunder. Thunderstorms are usually accompanied by
strong winds, heavy rain and sometimes snow, sleet, hail, or no precipitation at all and it can be
very destructive if the formation is severe.
e) Perhaps there are many other severe weather conditions but they are not applicable in Saudi
Arabia.
10.9.4. Responsibilities
Responsibilities during severe weather conditions will be consistent with normal line supervision
functions. Decisions as to what plan shall be followed, timing, curtailment, shutdown, etc. will be
solely decided by the Project Manager.
• Have all crane booms lowered to the ground or retracted to the shortest boom length.
• All cranes, derricks, gin poles, and erection rigs unable to be boomed down must be properly
grounded.
b) High Winds and/or Rain
• When high wind creates a hazard to craftsmen or work being performed (i.e., instability in
elevated areas, limited visibility due to dust or particles in the air, unmanageable materials, etc.)
supervision will stop work activities, re-assign work and area, properly store and secure material
which might blow away, injure or damage, lower/tie down crane booms and obtain further
instruction from Site Management.
• When wind speed reached 60 kilometers per hour (kph) no elevated works shall be allowed.
• Secure all loose objects and material, i.e., ladders, pallets, drums, trash in dumpsters, etc.,
• Set brakes and check all mobile equipment,
• Crane booms lowered down and secured,
• De-energize all unnecessary electrical power.
- When rain creates a hazard to craftsmen on work being performed.
- (i.e., un-stable footing conditions due to slippery structural steel or work platforms, muddy and
flooded work environments, unstable trenches or excavations, poor visibility due to rain or eye
protection) supervision will stop specific work due to hazard, re-assign work duties and/or areas,
and obtain further instructions from Project Management.
c) Thunderstorm
• All employees must seek shelter indoors whenever possible. When indoors, it is important that
employees avoid contact with electrical appliances and conductive surfaces and structures.
• Employees outdoor shall maintain a safe distance with the present conductive structure or object.
The safe distance shall be times two of the height of the conductive structure or object.
• Employees outdoor must avoid facilities or objects that may carry electric current from a
thunderstorm such as but are not limited to the following: telephone lines, extension cords,
pipelines, conductive structures and fences.
• When poor visibility is encountered, employees must remain at site and moves only when visibility
is improved.
d) Dust and Sandstorm
• All employees must wear a clear glass, dust mask and whenever they are having, a face mask,
to protect their eyes, faces, respiratory system and to sustain good visibility.

• No elevated works shall be allowed when the environment is in the estate of zero visibility.

property, materials, and equipment (i.e., rope wire boards/plywood).

- The wheel shall fit freely, but not loosely, onto the spindle. The clamping nut shall not be
tightened more than is required to hold the wheel firmly onto the spindle - Test runs the
new wheel at normal speed, standing well clear of the wheel during testing.
- Any guard shall be properly secured and adjusted, if the Machine is designed to be fitted
with a tool rest, it shall be adjusted to within 2 mm (1/8 inch) of the wheel. The guard shall
be capable of performing two functions. Firstly, to contain the wheel in the event of the
wheel bursting and secondly to prevent the operator coming into contact with the moving
wheel.
- The guard shall enclose the wheel as much as possible, given the design of the grinding
Machine and the nature of the task being carried out, and shall be firmly secured to the
frame of the Machine - Selecting the correct wheel will affect both the operational
effectiveness of the wheel and its safety. Generally, a soft wheel shall be selected for hard
materials and a hard wheel for soft materials. With an unsuitable wheel the face of the
wheel may become clogged with the material being ground (loaded or glazed).
- Truing and dressing is only required on stone or ceramic wheels. This shall be required
whenever the cutting action is reduced due to loading or glazing, or when the cutting edge
of the wheel becomes unevenly worn.
- Wheel breakage can occur when excessive pressure is applied to the wheel. Grinding on
the side of a straight-sided wheel shall be prohibited, unless the wheel has been
specifically designed for side grinding.
- In addition to other personal protective equipment that is mandatory on the site, persons
using grinding wheels, and those in the close vicinity, shall wear full, impact resistant face
shields, in addition to their safety glasses.
10.12. Illumination
10.12.1. General
- Contractor shall provide adequate lighting for employees to perform work during the ours
of darkness and in poor illuminated areas.
10.12.2. Requirements
- Construction areas, ramps, runways, corridors, offices, shops and storage areas shall be
lighted to no less than the minimum illumination intensities as listed below.
a. Feeder shall originate in a distribution center. The conductors shall be run as multi conductor
cord or cable assemblies or within raceways, or where not.
b. Subject to physical damage, they may be run as open conductors or insulators no less than
3.3 meters apart.
c. Branch circuits shall originate in a power outlet or panel board. Conductors shall be run as
multi-conductor cord or cable assemblies or open conductors or shall be run in raceways. All
conductors shall be protected by overcurrent devices at their capacity. Runs of open
conductors shall be located where the conductors will not be subject to physical damage and
the conductors shall be fastened at intervals not exceeding 3.3 meters.
d. No branch circuit conductors shall be laid on the floor. Each branch circuit that supplies
receptacles or fixed equipment shall contain a separate equipment grounding conductor if the
branch circuit is run as open conductors.
e. Receptacles shall be of the grounding type. Unless installed in a complete metallic raceway,
each branch circuit shall contain a separate equipment grounding conductor and all
receptacles shall be electrically connected to the grounding conductor. Receptacles for uses
other than temporary lighting shall not be installed on branch circuits, which supply temporary
lighting. Receptacles shall not be connected to the same ungrounded conductor of multi wire
circuits, which supply temporary lighting.
f. Disconnecting switches or plug connectors shall be installed to permit the disconnection of all
underground conductors of each temporary circuit.
g. All lamps for general illumination shall be protected from accidental contact or breakage. Metal-
case sockets shall be grounded.
h. Temporary lights shall not be suspended by their electric cords unless cords and lights are
designed for this means of suspension.
i. Portable electric lighting used in wet and/or other conductive locations, for example, drums,
tanks and vessels shall be operated at 24 volts or less.
j. A box shall be used wherever a change is made to a raceway system or a cable system, which
is metal, clad or metal sheathed.
k. Extension cord sets used with portable electric tools and appliances shall be of three-wire type
and shall be designed for hard or extra hard usage. Flexible cords used with temporary and
portable lights shall be designed for hard or extra hard usage.
l. Temporary wiring shall be removed in its entirety immediately upon completion of construction
or the purpose for which the wiring was installed. In isolated cases, where removal is
impractical and with management approval, temporary wiring may be abandoned in place. In
such cases, ends of wires or cables shall be disconnected from the power source, taped and
tagged as abandoned.
11 Health, Sanitation & Welfare
First Arab has an established Camp, Catering and Facility Management in-placed and the same will
be executed for the administration of this requirement for this project.
11.1. Toilet
Adequate portable toilets will be provided in work areas and work areas where they may be easily
accessible at the rate of one toilet per fifteen workmen (1: 15). First Arab Management will ensure
that all toilet facilities used by employees are cleaned and serviced on a scheduled basis that will
prevent the facilities from becoming unsanitary and unusable. Each portable toilet facility shall
include both a toilet seat and a urinal.
11.4. Housekeeping
Utility men/housekeeping crew shall be assigned during mid-day meal for housekeeping. All waste
food and garbage shall be disposed daily.
11.5. Waste Disposal
All construction /General waste shall be collected, segregated and stored on regular basis and will
be disposed only to approved Waste Disposal location by SIPCHEM.
Waste Skips shall be provided for collecting each type of waste generated at the site.
Removal of waste from the site shall be monitored by the dedicated supervisor to avoid overflowing
of waste at the site.
Records of Waste disposing shall be kept at stores for future reference.
NOTE: Hazardous and sewage waste handling, storing and disposing procedure is added in the
document under Waste Management section.
Biological waste shall be collected in the BIO-HAZARD (Yellow colored) disposable bag and the
transfer it to the approved clinics for proper disposal
All of our employees were undergone pre-employment medical checkup to prove their fitness to
work, prior to join with the company.
Emergency contact numbers shall be pasted at the site office for report any unsafe conditions or
actions or incidents by initiating reporting protocols.
First Arab project team shall report all near miss, first aid, accidents or environmental incidents,
property damage and ill health of project employees immediately to the client.
Initial Incident notification report shall be submitted within 24 hours by using approved reporting
format to the client project engineer, safety Engineer.
All incident investigation report shall be prepared by the respective department engineer who will be
assisted by safety team to determine and implement the control measure at the site.
A Project investigation team consisting of the construction manager, HSE Officer / Representative
and respective Contractor superintendent and supervisor(s) shall jointly investigate accidents,
incidents, near misses and first aid cases.
Incidents will be reported the same day within two hours of occurrence by phone to the SIPCHEM
HSE Engineer and the project management. First Arab Accident Report Form shall be used for
reporting and / or SIPCHEM accident report format. Incident report shall be immediately given to
contractor within twenty-four (24) hours and final report to follow within three (3) days.
The following definitions will apply to incident and accident reporting and investigation:
a. Accident – Means an activity that result in an injury to one or more persons.
b) Average Lost Time – Means the average time lost for every Lost Workday Case Injury. This is a
measure of severity of the injuries and is calculated as follows: Average Lost Time = Number of
working days lost divided by the number of LWDCs in the period.
c) Contributing Root Cause – Means a factor, event, or circumstance which led, directly or
indirectly, to the dominant root cause, or which contributed to the severity of the Incident or Near
Miss.
d) Corrective Actions – Means changes to design processes, work instructions, workmanship
practices, training, Inspections, tests, Procedures, specifications, drawings, tools, equipment,
facilities, resources, or material that result in preventing, minimizing, or limiting the potential for
recurrence of an Incident.
e) Cost of Incident – Means the direct cost of replacement of damaged equipment and parts, plus
labor, as well as cost of clean-up and any environmental investigation activity and restoration of
property as required by Environmental regulations.
f) Dominant Root Cause. - Means along a chain of events leading to an Incident or Near Miss, the
first causal action or failure to act that could have been controlled systemically either by
Policy/practice/Procedure or individual adherence to Policy/practice/Procedure.
g) First Aid - First aid involves initial treatment and subsequent observation of work-related minor
scratches, cuts, burns, splinters, etc., which (1) do not ordinarily require professional medical
care even though the treatment may be provided by a physician or registered professional
personnel and (2) do not involve loss of consciousness, a lost workday, restriction of work or
motion, transfer to another job.
The following Procedures are considered first-aid treatment.
- Application of antiseptics during first visit to medical personnel. Treatment of first degree burn(s);
- Application of bandage(s) during any visit to medical personnel. Use of elastic bandage(s) during
first visit to medical personnel;
- Removal of foreign bodies not embedded in eye if only irrigation is required;
- Removal of foreign bodies from wound if Procedure is not complicated and is, for example, by
tweezers or other simple technique;
- Use of non-prescription medications and administration of single dose of prescription medication
on first visit for minor injury or discomfort;
- Soaking therapy on initial visit to medical personnel or removal of bandages by soaking;
- Application of hot or cold compress(es) during first visit to medical personnel;
- Application of ointments to abrasions to prevent drying or cracking;
- Use of whirlpool bath therapy during first visit to medical personnel;
- Negative X-ray diagnosis; or
- Observation of injury/illness during visit to medical personnel.
h) Hazardous Condition – Means any situation that exposes Workers to a physical, chemical,
electrical, high energy, etc. hazard that may adversely affect their health or safety
i) Hazardous Occurrence- Mean an event that has resulted in an injury or negative impact to
property.
j) Hazardous Product – Means any product that is prohibited, restricted or controlled.
k) Incidence Rate - Is defined as the number of injuries per 200 Workers. The frequency rate is
calculated as follows: Number of injuries in the period x 200 divided by the number of Workers.
l) Incident – Means an unplanned event those results in damage to public or private property.
m) Lessons Learned – Means knowledge or understanding gained by experience. The experience
may be positive, as in a successful operation, or negative, as in an Incident or failure. A lesson
must be significant in that it has real or assumed impact on operations; valid in that it is factually
and technically correct; and applicable in that it identifies a specific design, process, or decision
that reduces or limits the potential for failures and mishaps, or reinforces a positive result.
n) Lost WORKDAY CASE (LWDC) - Is defined as a nonfatal traumatic injury that causes any loss
of time from work beyond the day or shift on which it occurred; or a nonfatal non-traumatic
occupation illness that causes loss of time from work or disability at any time as defined on the
current medical certificate.
o) Lost Workday Case Frequency Rate LWDFR - Is defined as the number of injuries per two
hundred thousand hours worked. The frequency rate is calculated as follows: Number of lost
time injuries in the period x 200,000 divided by the number of hours worked in the period.
p) Medical Treatment – Means an injury that requires treatment at a medical facility that cannot be
administered as First Aid onsite.
q) Medical Treatment Case MTC - Is defined as any work-related loss of consciousness, injury or
disease requiring more than minor First Aid treatment by a medical practitioner or registered
medical personnel but not resulting in lost time or alternate /restricted duties. The following
Procedures are considered Medical Treatment. Any Contractor work-related injury/ illness for
which the following type of treatment was provided or should have been provided is considered
to be a Medical Treatment Case:
o Treatment of infection. Application of antiseptics during second or subsequent visits to
medical personnel;
o Treatment of second or third-degree burn(s);
o Application of sutures (stitches);
- Application of butterfly adhesive dressing(s) or sterile strip(s) in lieu of sutures;
o Removal of foreign bodies embedded in the eye;
- Removal of foreign bodies from wound if Procedure is complicated because of depth of
impediment, size, or location;
- Use of prescription medications (except a single dose administered on first visit for minor
injury or discomfort);
- Use of hot or cold soaking therapy during second or subsequent visit to medical personnel;
- Application of hot or cold compress(es) during second or subsequent visit to medical
personnel;
- Cutting away dead skin (surgical debridement). Application of heat therapy during second or
subsequent visits to medical personnel;
- Use of whirlpool bath therapy during second or subsequent visit to medical personnel;
- Positive X-ray diagnosis (fractures, broken bones, etc.); or
- Admission to a hospital or equivalent medical facility for treatment (not merely observation).
r) Near Miss - Is defined as a situation or occurrence with no injury, no damage but possesses the
potential to cause any type of Incident, or any injury, damage, or negative impact. (A Near Miss
is not considered an Incident, but the Incident reporting, investigation, and record keeping and
recurrence control guidelines will be followed.)
s) Occupational Illness – Means any abnormal condition or disorder of an Employee (other than
one resulting from an Occupational Injury), caused by exposure to environmental factors
associated with employment.
t) Occupational Injury - Means any injury that results from a work Accident or from exposure to
environmental factors associated with employment.
u) Recordable Injury – Means Fatalities, Lost Time, Medical Treatment and restricted work injuries
are referred to as Recordable injuries.
v) Restricted Work – Mean an injury, often stemming from a medical aid injury that prevents the
Worker from returning to his regular job. Also, referred to as light duty.
w) Restricted Work Case (RWC) - Is defined as an injury or Occupational Illness that results in the
injured person (a) being assigned to another job; or (b) working at their permanently assigned
job with some restrictions on duties; or (c) working at their permanently assigned job with some
restrictions on their working time for periods of a complete shift or day or more as defined on the
current medical certificate.
x) Root Cause Analysis – Is a structured process for identifying the basic factors, reasons, and
causes for conditions that result in Incidents or Near Misses. Once identified, the conditions can
be corrected and future Incidents or Near Misses prevented.
y) Total Reportable Injuries (TRI) – Means the following injury classifications, i.e. lost WORKDAY
CASES, Medical Treatment injuries, restricted work injuries and fatalities.
z) Total Reportable Injury Frequency Rate TRIFR Is defined as the number of injuries per two
hundred thousand hours worked. The frequency rate is calculated as follows: Number of OSHA
recordable injuries in the period x 200,000 divided by the number of hours worked in the period.
12.1. General
a.) The purpose of any accident investigation is to identify all possible contributing causes so that
future incidents, similar in nature, can be prevented and to determine all the facts which may
have a bearing on legal liability. Investigations should be directed toward fact-finding, not fault
finding.
b.) His investigation should begin as soon as possible after the necessary notifications have been
accomplished.
12.1.1. Conducting the Investigation
a.) When possible, discuss the accident with the injured employee.
c.) Discuss the accident with other employees who may have seen the accident.
d.) Carefully consider the following points:
i. What was the injured employee doing prior to, and at the time of, the accident? Was this in
pursuit of his regular duties?
ii. Was the employee properly instructed as to the manner in which to perform his duties? Did
he/she do the work in accordance with instructions?
iii. Did any other employee or contractor contribute to this accident?
iv. Was the equipment or Machinery which the injured employee was using in good condition?
Was it properly guarded? Was it suited for the purpose for which it was being used?
v. Was ample and sufficiently lighted work space provided?
vi. Were proper housekeeping conditions maintained?
vii. How is the same type of work done by other employees?
viii. Is there a safer way in which this work could be done?
ix. Was the injured in good health when reporting for work on the day of the accident?
12.1.2. Witnesses
a.) All personnel associated with the operation and other eye witnesses to the accident shall be
interviewed and written statements taken.
b.) The information obtained during these interviews must be limited to direct knowledge of what
was observed. Opinions and hearsay information do not represent factual findings.
c.) Each individual interviewed should be requested to sign a statement of his recorded sequence
of events that leads up to and include the accident.

d.) The following information should be obtained from each individual interviewed:
i. Name, employing contractor, employee number, address and occupation or trade.
ii. i Date, time and place of interview. Where the person being interviewed was at the time of
the accident.
iii. A complete narrative of what the witness knows of the accident.
iv. What operational activity or other events were taking place prior to and at the time of the
accident.
v. What materials (lumber, concrete, steel, etc.), equipment (tools, cranes, scaffolding, etc.)
or conditions (weather, working environment, labor disputes, etc.) were involved? This
would also include all possible contributing factors, personal and physical, whether they
are directly or indirectly related to the accident.
vi. What facts may have caused the accident? Answers must be as objective as possible.
Include all unsafe conditions and unsafe acts.
vii. Was there a pre-existing known and/or reported unsafe condition or reported, to whom
and was there any action taken at that time.
e.) Conclusion
Upon conclusion of the interview, review the statement with the witness and attempt to clear up
possible discrepancies. The statement should then be dated, signed and witnessed by a third party.
A sample form of a witness statement is included at the end of this section.
12.1.3. Evidence
a.) It is in the best interest of all parties that all physical evidence not be disturbed or tampered
with, regardless of the circumstances involved.
b.) All efforts must be made to secure the area of the accident as soon as possible after the
occurrence to prevent any alteration of the scene prior to the investigation.
c.) If any equipment, tools and/or materials are involved with the accident, they shall, after marking
location, be removed from service and placed in safekeeping. If this proves to be impractical,
the area in which the accident occurred shall be cordoned off and security personnel shall be
posted to keep all unauthorized personnel out of the area.
d.) The secured area shall only be reopened upon approval from the SIPCHEM HSE Manager.
12.1.4. Photographs, Video Recordings, Drawings and Diagrams
a.) The photography shall be taken prior approval from SIPCHEM as per their procedure and
authorize camera shall be utilized with the SIPCHEM approval based on the requirement,
Where applicable and with proper authorization, photographs may be taken of the scene of the
incident as well as any equipment involved in the incident. If possible, sufficient photographs,
and/or video recordings shall be taken as soon as possible after the accident by the Safety or

designees, (if authorized), since conditions rapidly change. Each photograph shall be properly
labeled with the following information:
i. Description and location of principal item(s),
ii. Positions/directions in which the photographs were taken,
iii. Dates and time and,
iv. Name of photographer.
b.) The investigator should endeavor to provide a series of shots which supply a maximum of
useful information and which will enable the viewer to understand how the accident occurred.
c.) Several photographs should be taken employing a general view. The SIPCHEM approved and
authorized camera should be utilized clockwise until at least four general view photographs
have been taken.
d.) A set of views should be selected to show the relationship of the accident to surrounding
structures or articles. This may be suggestive of action immediately preceding the accident.
e.) As the scene is examined, various objects will appear to have relation to the injury. Tools,
blood stains and similar items should be photographed before they are moved or cleaned up.
Two (2) photographs are needed for a significant object, which is less than 300 mm in length.
The first should be at close range to obtain a fairly large image of the article. Also, the use of
a ruler laid alongside of the object shows exact Measurement of object. The second
photograph should be taken with the camera approximately 1.8 meters from object in order to
bring the background in view and show the object in perspective.
f.) Any contiguous areas, which may have been used by victim, should be considered part of the
scene of the accident. The nature of the accident will determine the extent to which the environs
need be photographed for a fuller understanding of the events that lead to the accident.
g.) The camera should be carefully placed to provide a perspective, which is both normal and
informative. The incorrect selection of photographic angle often results in a distorted and false
impression of the scene. This must be avoided, as we want to represent only factual
information.
h.) It is sometimes desirable to illustrate the statement of a witness by means of a "posed"
photograph. In this way, the inadequacies of verbal testimony can be graphically remedied. To
accomplish a posed photograph, a person with the same general physical appearance should
be used. Naturally, he should be placed in the same spot and positions as directed by the
witness. Marks or pointers should be used to clarify important aspects of the photograph. For
example, chalk can be used to show body position of victim. Prior to such a procedure,
however, photographs should be made of the untouched scene to obviate any objection to the
photograph in court on the grounds that it does not purport to show the original scene.
i.) Drawings and diagrams should be marked up and/or sketches prepared to indicate the location
of the accident. All measurements of time, distance, size, weight, etc., must be accurate. In
the event of unknowns (speed, distance, weight, etc.), every attempt must be made to closely
approximate the same with tables, formulas or calculations which must be kept as part of the
accident investigation.
13 Demolition Works
The work that will be demolished in the existing Car Parking Shed, Street Light, Green Islands will
be transported to the First Arab Dammam Yard. During demolition work, the main demolition areas
must be ready to start our activity. Suitable safety signage must be posted. Dust suppression
measures must be employed throughout the works.
13.1.2. Services
First Arab will establish a formal procedure to ensure that any unrecorded services are located during
the works which are carefully checked to determine their nature and status; to record relevant
information and provide to SIPCHEM Project Manager prior to agreeing a course of action.
Measures shall be taken to protect all services as specified prior to commencing the demolition
works.
First Arab will take all measures necessary to protect others from any risks arising out of the works,
wherever possible to explain the works and sequences of operations to allow for the complete
segregation of all demolition activities, Wherever not possible to explain the hazards and risks which
will affect others and take appropriate measures, including the provision of hoardings/barriers,
protection from falling materials, avoidance of uneven surfaces, adequate illumination, etc., to
adequately control the risks.
First Arab to ensure all vehicles leaving the site with loaded demolished materials and shall be
suitably covered to ensure no dust, debris or rubbish may be blown or fall from the vehicle during its
route.
Dust suppression measures must be employed throughout the works and noise must be kept to
minimize possible for this type of work.
First Arab will provide an adequate method statement to demonstrate its intentions.
The following activities have been identified as posing a special health & safety hazard during the
demolition phase of the works.
14 Hot Work
All Hot work activities shall be covered by an appropriate SIPCHEM permit. Particular care will be
taken while carrying out hot work at all times and especially on the locations where combustibles are
available.
14.1. Instructions
a) Fire watcher and required firefighting equipment must be available prior to the operation.
b) Precautions such as inspection of the surrounding area, removal of any combustible
material, protection by fire blankets and provision of fire extinguishers.
c) Fire extinguishers shall be available before the commencement of hot work and people shall
be trained to their correct use.

15 Cold Work
When work permits are required, the purpose of the cold work process is to control work activities
that will not produce sufficient energy to ignite flammable atmospheres or combustible materials.
A cold work permit applies to work that can still involve hazards that must be evaluated during the
joint site inspection.
Cold work can still involve hazards that may require conducting atmospheric gas tests. Workers may
be required to use specific personal protective equipment (PPE), erect barricades and follow specific
work procedures.
16 Excavation
Excavation accidents are caused by contact with underground utilities, by collapse of trench or
excavated walls, by falls of equipment and persons, by excavated equipment striking personnel, by
hazardous atmosphere and inadequate shoring.
Safety Requirements
Before the job starts, the following factors must be considered:
• Prior to excavation, drawings shall be checked by contractor/sub‐contractor to determine if there
are any underground installations pipes, cables exist. These shall be protected against damage.
• Maintain sufficient angle of repose. Provide slope not less than 1:1 and suitable bench of 0.5m
width at every 1.5m depth of excavation in all soils except hard rock.
• As far as possible, excavated material should not be placed within 1 m from the edge of
excavation or depth of excavation whichever is greater.
• Barricades, warning lights shall be erected at 1 m from the edge of excavation adjacent to
pedestrian or vehicular traffic.
• Two safe access / exits shall be provided by means of ladders, stairs ramps for excavation
beyond 1.5m.
• Where vehicles or Equipment operate near excavations, the side of the excavation is made to
withstand the forces exerted by the superimposed load.
• Each excavation is checked daily. If there is any cave‐in or slide, all the work in the excavation is
stopped forthwith.
• Don’t allow vehicles to operate too close to excavated area. Maintain at least 2m distance from
edge of excavation.
Method of excavation
• Obtain Work Permits from SIPCHEM
• Provide Excavation plan to include depth of cut, location, and underground utilities to Client
Representative.
• Client Representative approval is required on all sections to be excavated;
• Excavations are to be barricaded with reflective tape at all times and appropriate signage shall
be provided.
• All excavations below four (4) feet are to be shored or laid back to a safe angle (slope) where
needed;
• Excavated materials and equipment to be kept at least three (3) feet back from edge of
excavation;
• Provide trench cross-over for workers (sturdy planks with hand rails);
• Spoil materials (soil) not to block roadways, etc. Haul to safe dump area.
• Backfill those portions of the excavation where work is completed;
• Provide pumps on flooded conditions. No work is to be accomplished in flooded excavations.
SIPCHEM Representative’s approval is required;
• Provide warning lights at night and it shall be fixed system lights, not flashers.
The purpose of the confined space work is to ensure safe entry by personnel when any part of the
body enters into a space that meets the criteria for a confined space. A CSE permit applies to work
during entry into any space or structure that:
• Has limited or restricted means of entry or exit.
• Is not designed for human occupancy.

Examples of confined spaces include tanks, vessels, vessel skirts, vaults, manholes, sewers, valve
boxes, lift stations, and structures or excavations 1.2 m (4 ft) deep or deeper. Areas above floating
roof tanks where the top of the roof is more than 1.2 m (4 ft) below the rim of the tank are also
considered confined spaces.
The Company will ensure that proper heat stress awareness training shall be provided just before
and during the hot summer months to all personnel especially those working outdoors. These include
the personnel who manage and responsible for heat stress measures.
Heat stress awareness includes hazards, recognition of signs and symptoms, employee’s
responsibilities to heat-related illness, first aid procedures for heat stroke person, dangers of used
drugs, which include prescription and over-the-counter medicines.
Supervision should be in place in order to avoid and respond immediately to heat related illness.
Provide measures for employees can take to minimize the effects of heat stress, which include
hydration, work pace, good nutritional habits, use of wet towels, etc. Personal monitoring and buddy
system should be implemented.
Responsibilities
Site Engineer, Supervisors and Foremen must ensure that: When heat-related illness exists, always
monitor environmental conditions (e.g., air temperature, relative humidity) and communicate the
current heat stress danger category and corresponding control measures to relevant Saudi Aramco
and employees.
Document heat stress mitigation measures in the specific work plan, which shall specify the specific
work practices to be implemented at the site to avoid heat-related illness during periods of high heat
stress potential and may include:
• The scheduling the most strenuous work during the coolest times of the day to prevent heat
illness (e.g., early morning and evening/night).
• As much as possible, minimize the work time during high degree of sunlight.
Do not let schedule or productivity demands supersede heat stress awareness or controls used to
prevent heat-related illness.
Regularly conduct periodic (e.g., weekly) safety meetings/talks during hot weather emphasizing heat
stress hazards and precautions, with added emphasis on this risk during Ramadan if it occurs in
summertime.
Schedule heat stress training and monitor new workers and workers returning from vacation to
ensure they acclimatize at a progressive, controlled rate to the change in environmental conditions.
Provide drinking water stations (e.g., coolers with chilled or ice water) for workers and remind them
to drink plenty of water even if not thirsty. Replenish drinking water supplies as needed to ensure
availability and provide more frequent collection of sanitary wastewaters from portable toilets.
Provide personal water bottles or insulated containers (2-liter capacity) to workers as needed (e.g.,
during high heat stress danger categories).
Provide designated shaded and cool areas for periodic “cool down” breaks and recovery from minor
heat-related illness. Where possible, these areas are to be airconditioned.
Display safety information related to heat stress conditions, illness, and preventive measures in
bulletin boards, resting area etc. Distribute heat stress information thru toolbox and safety meeting
which suitable to all workers.
Monitor the heat-related health condition of employees, as well as the effectiveness of any
engineering/administrative controls and personal protective equipment (PPE) being used.
Be able to recognize early signs and symptoms of heat-related illness and take appropriate action to
prevent serious heat illness.

I. Engineering Controls
Shaded Area
Designated shaded break/rest areas shall be provided at a distance not greater than 100 m (330 ft.)
from personnel working in direct sunlight for extended periods of time. When feasible, shade is to be
provided for work areas, including single shades to accommodate flagmen, etc.
Local Ventilation
When the air temperature is less than 37 °C (99 °F), fans or portable air movers may be used to
reduce heat stress by increasing the airflow in the work area.
Portable Air-Cooling Systems
Portable blowers with built-in air chillers shall be used as needed (e.g., when confined spaces and
enclosed work areas have ambient temperatures exceeding 43 °C [110 °F]) to provide cooling for
confined spaces and similar enclosed work areas.

• Red Heat Stress Danger Category IV Work Practices. The following additional precautions
shall be taken while a red heat stress danger category IV, “Extreme Danger,” (see below)
exists at the work site. Heat stress shall be indicated and communicated by displaying
YELLOW (Low Heat Index) and RED Flag (Higher Heat Index level – STOP WORK) on the
pole post.
19 Waste Management
This procedure applies to the management of waste at construction of Car Parking Project at
SIPCHEM during its initial generation through the final disposal for all areas of works including
offices, and construction sites during the construction and demobilization activities under scope of
First Arab and its Subcontractors in order to:
• Minimize the potential to cause harm to human health and the environment;
• Achieves and maintains compliance with Company Environmental Regulations;
• Avoid severe environmental impact of waste. Impact on ecology, aesthetic and visual quality.
• Pollution of soil, air & water resources
• Foul odors & nuisance due to decomposition & disposal of waste
• Occupational health hazards
First Arab and its subcontractors are responsible for keeping their work area clean and for removing
waste every day. First Arab and its Subcontractors will provide waste containers in their respective

areas and these will be emptied. Wastes covered by this section are types of waste, which can be
delivered to client, approved waste handling sites. The sources of wastes that are expected to be
generated in the project are:
• Domestic Wastes
• Construction Wastes
• Hazardous Wastes
Hazardous wastes expected from the project, but not limited to, are as follows:
• Waste chemicals
• Waste Lubricants
• Oily Sand
• Tank & Other sludge
• Batteries
• Tires
• Oil Drums
• Medical Waste
• Other waste
Gallon drum containers will be provided by the existing facility for collection and storage onsite of
hazardous waste. The container will be labelled “HAZARDOUS WASTE”.
Clinical waste will be collected from the site medical facilities and from the mobile first aid units on
daily basis. Sharps and needles will be disposed of in special sharps containers, after being sterilized
by an autoclave. All medical waste will be placed inside tagged yellow bags, and separated from
other types of waste on the project. Furthermore, clinical waste containing sharps and needles will
be segregated from medical waste consisting of solid dressings, and tissue.
Bodily fluids will be treated with disinfectants before being disposed. Waste chemicals and lubricants
will be collected in compliance with the suggestions and recommendations of the specific Material
Safety Data Sheets (MSDS).
Storage
Storage, use, and disposal will be in accordance with the OSHAS standards CFR 1926 Subpart F
926.150,151,152 “Waste Management” and SIPCHEM Environmental Regulations.
✓ Hazardous waste will not be stored on-site for more than 90 days.
✓ Hazardous waste material will be stored in a separate storage area specifically designated for
this purpose. The area will have an impervious surface with a suitable drainage control,
roofing and be dike surrounding the perimeter of the storage area.
A non-permeable ground covering will be placed in the storage area floor to prevent spills from
contaminating ground soil.
Special precautions will be taken to treat surface run-off from this area, and also for the disposal of
fire water Quantities stored will be kept to an absolute minimum. The monthly average contents will
never exceed 75% of design capacity.
All hazardous waste will be properly stored. Containers will be marked with warning labels to
accurately describe their contents and detail appropriate safety precautions. The storage area
perimeter will also be identified by orange safety netting.
Incompatible hazardous wastes will not be stored in the same storage or transportation container.
Hazardous chemicals will be stored and handled in accordance with the Manufacturer’s Material
Safety Data Sheet (MSDS) or as per Company Environmental Regulations.
First Arab and its subcontractor will have readily available all the relevant MSDS’s at the Chemical
Storage Area and the location where chemicals are being used. The project procurement Manager
will be responsible to acquire all the MSDS’s from the Suppliers.
Fire Extinguishers rated not less 20AB will be mounted no closer than 8 meters, and no further than
24 meters from flammable, liquid, and hazardous waste collection storage area.
Transport
The hazardous waste will be stored in the appropriate containers and will be transported to the proper
disposal locations.
✓ The vehicles will be marked “Hazardous Wastes – Improper Disposal Prohibited”.
✓ The driver will be provided with copies of the MSDS relevant to the material he is transporting
in the vehicle
✓ The driver will also have a copy of the waste manifest which will indicate source of waste,
type of waste, quantity, disposal method and location and date and time.
✓ The client will be informed of any hazardous waste transport in advance prior to disposal.
Disposal
Hazardous waste (Class 1) will not be disposed directly to the municipal and fill; co-disposable with
municipal waste is technically acceptable provided that it meets the requirements of the Local
Regulatory Authorities.
All hazardous waste will be disposed to SIPCHEM approved waste management contractor wherein
a complete list of these approved contractors are maintained on the Environmental Protection
Department website.
20 TRAINING PROGRAM
Scope
First Arab will develop site safety training program to ensure all workers and staffs improve their
knowledge and skills towards the safety requirements and procedure in the site specially SIPCHEM
HSE and requirements, HSE program and plan. To prevent any possible unexpected accident,
incident, near misses in the workplace. People involved but not limited to Project line managers,
engineers, supervisors/foremen and office staffs, site skilled workers and laborer’s, Sub-Contractors
who have been working within the organization etc. (see below specific categories).
Limitation
Specific trainings that is listed on the matrix training program which is highly needed in the project
prior to execute the work. First Arab will notify 3rd party training provider to conduct and execute the
training.
Internal Training
a. Safety First Arab Trainers will do the internal training
b. Develop training materials (if necessary) and seek approval from the department managers
prior to the implementation.
c. Choose the training methodology to be used.
d. Source out and evaluate external trainer who will conduct an in – house training, if there is no
qualified Internal trainer.
e. Furnish a copy of the approved training schedule to all departments’ heads and sections
concerned.
f. First Arab will be using a dedicated furnished training room with projector in site for all
training and induction purposes.
In an effort to provide a safe and healthful workplace, trainings will be provided to all First Arab,
subcontractor’s employees along with visitors. The safety training will emphasize risks that are
present and controls that are in the workplace. The First Arab employees shall participate in the
training programs that are required for their job category. An employee who feels that more
information is needed shall inform his site supervisor / HSE officer /foreman for further trainings. The
site Safety
Manager will deliver the Trainings.
Identify the training needs and ensure that entire workforce has necessary knowledge and skills to
carry out their activities. Enable all personnel to reach their full potential. Improve efficiency and
effectiveness of all company activities. Enable new techniques and skills to be introduced in a timely
manner.
This practice provides the requirements for project- and office-specific Health, Safety, and
Environmental (HSE) orientations and training for First Arab Company Limited personnel, Sub-
Contractors personnel, vendors and visitors. The training is to be adapted to offices, project, and
site-specific conditions.
Please refer the Attached Safety Training Matrix for various training courses delivered by First Arab.
All these courses attached would be in-house training. The safety training courses required for each
job category are mentioned in the Training Matrix.
Mandatory Training
All listed in the training matrix which is most required, likes; Safety Orientation, Workers vehicle
interface, Defensive Driving etc.
SAFETY ORIENTATION
All employees and the Sub-Contractors in the project shall be required to attend safety orientation
meeting before starting their works. Orientation explains about the plant, production, health &
environmental hazards, scope of work and its related activities, work location, access of work area
etc. This shall be mandatory for each employee new or longstanding, coming to work on a new site.
This orientation will deliver in different languages. New comers will be reporting to HSE officer
through administration department to get project orientation done. New employee / sub-contractor
orientation will be for at least 20 -30 minutes. Visitors and suppliers will have quick orientation for 10
minutes. Without orientation, no employees (First Arab or Subcontractors) are allowed to work in the
project site.
Risk Assessment
Task Specific Risk Assessment shall be prepared and submitted along with Method of Statement
through submittals for getting approval from SIPCHEM, prior to commence of Work activity.
