INCIDENT LEARNING: Line of Fire Injury – IP struck by reversing equipment


Outcome: Lost Time Injury (HIPO) When: October 2023 Where: ZMC, Abu Dhabi
WHAT HAPPENED?
• Contractor employees were working in a small area (48m2) as part of
a new substation construction project
• A 12-tonne wheeled loader was being used to move and level sub-
base material across the worksite
• A worker (IP) was kneeling on the ground installing levelling markers
with his back to the loader
• The IP was struck by the reversing loader and sustained chest injuries Type of wheeled loader
involved in the incident
and a pelvic fracture, requiring surgery and extensive recovery

THE CONTEXT
• The absence of a thorough risk assessment meant that potential hazards relating to heavy
machinery operations were not adequately identified or effectively controlled for the small worksite
• An effective Traffic Management Plan was not implemented to ensure adequate separation
between workers and moving equipment
• Site supervision did not enforce site safety protocols and allowed the loader to operate whilst
personnel were working in the area
• The assigned flagman (spotter) failed to spot the IP in the line of fire; the operator’s view was
obstructed by a sunshade placed across the side windshield and failed to see the IP
• Barriers did not prevent the IP from entering the site whilst the loader was in operation
• Inadequate training and awareness was provided to the workforce regarding safe operation in
proximity to heavy machinery and the proper implementation of safety protocols
• Poor safety leadership, communication and lack of attention to safety protocols was evident

LESSONS LEARNT
The incident serves as a reminder of the importance of the following safe work practices:
• Always take time to properly plan and risk assess work activities, ensuring suitable equipment is
selected for the task and simultaneous operations have been considered
• Active onsite supervision is vital to ensure protocols are understood, followed and timely actions are
taken to manage any unsafe acts or unsafe conditions that may occur
• Effective communication of potential hazards is essential – Toolbox talks and last-minute risk
assessments ensure every member of the work party is aware of what could cause harm
• Where site access controls and barriers are used, they must be effective in preventing unintentional
exposure to hazards
• Stay out of the Line of Fire - Never put yourself in the direct path of moving equipment
• Stay vigilant when working in close proximity to mobile equipment - Remain alert to unexpected
changes in their operation and movements
• STOP work whenever you think something is unsafe or the work changes – This may be the last
chance to prevent an accident
