DATA PRIVACY CONSENT FORM

In compliance with the Philippine Data Privacy Act of 2012 (RA 10173), your consent may be required for the LSERV
Corporation [hereinafter called the “Company”) to process Personal Data including personal identifiable and sensitive
personal information [collectively called “Information”] that it may have in its possession. By signing this form, you are
providing permission for the Company to process your “Personal Data” for the purposes of employment and deployment
to clients.

Personal Data may include information on your name, address, contact details, resume, details of birth, education
background, employment history, and results of employment checks (including background, credit and references
checks), copies and other details of identification documents, marital status, racial or ethnic origin, physical or mental
health or medical condition(s), commission or alleged commission of any offence or proceedings for any offence
committed or alleged to have been committed including past misconduct, the disposal of such proceedings or the
sentence of any court in such proceedings and information captured on security systems (including voice, video, Close
Circuit Television (CCTV) and security recordings), including personal financial and social security details, your relatives’
personal and financial details, and your emergency contact details.

Any Personal Data including additional information you may subsequently provide to the Company may be
collected, recorded, organized, stored, updated, modified, retrieved, consulted, used, and consolidated for the purposes
of the contract of service or such other contract as may have been entered into between the Company and you for the
purpose of employment.

In addition to above mentioned purpose, your Personal Data and other Personal Data that may be subsequently
collected including sensitive personal information may be collected, recorded, organized, stored, updated, modified,
retrieved, consulted, used, and consolidated by the Company for the following reasons:

a) processing employment applications including pre-employment checks;

b) determining and reviewing salaries, incentives, bonuses and other benefits;

c) consideration for promotion, career development, training, secondment or transfer, performance

monitoring, health and safety administration and security and access control;

d) monitoring compliance with our internal rules and policies and adherence to our business conduct
guidelines;

e) complying with the compliance and disclosure requirements of any and all governmental and/or
quasi-governmental departments and/or agencies, regulatory and/or statutory bodies;

f) to enforce our rights under applicable laws to defend our rights under the laws including but not
limited to any disciplinary action or actions relating to the termination of employment; and

g) purposes relating thereto.

Employees and potential employees ought to be aware that failure to supply such information will result in us being
unable to process employment applications, or affect our ability to perform our obligations under any potential or
existing employment contract and/or your ability to enjoy the benefits of your employment.

Personal Data held by the Company relating to employees and potential employees will be kept confidential but
the Company may provide such information to the following parties, for the purposes as set out above:

a) any other person under a duty of confidentiality to the Company which has undertaken to keep such
information confidential;

b) any person to whom the Company is under an obligation to make disclosure under the requirements of
any law binding on the Company or under and for the purposes of any guidelines or regulations issued by
regulatory or other authorities with which the Company is expected to comply;

c) any court and/or officer of the court;

a) sinumang iba pang tao sa ilalim ng isang tungkulin ng pagiging kompidensyal sa Kumpanya na gumawa na
ng mga hakbang upang mapanatiling kumpidensyal ang naturang impormasyon;

b) sinumang tao na ang Kumpanya ay may obligasyon na magsagawa ng pagsisiwalat sa ilalim ng mga iniaatas
ng anumang batas na sumasaklaw sa Kumpanya o sa ilalim at para sa mga layunin ng anumang mga
alituntunin o regulasyon na inisyu ng isang regulatory body o iba pang mga awtoridad na inaasahan ang
Kumpanya na sumunod;

c) anumang hukuman at/o opisyal ng korte;

d) anumang propesyonal sa pangangalagang pangkalusugan o ospital;

e) ang aming mga auditor;

f) ang aming mga abogado at mga compliance professionals;

g) ang aming mga bangko; at

h) aming mga kliyente

Ang Kumpanya ay mayroong karapatan na baguhin ang Kasulatan ng Pahintulot na ito sa anumang oras at matiyak
na ang mga naturang pagbabago ay maipaparating sa iyo sa pamamagitan ng anumang paraan na aming itinuturing na
angkop at naaakma.

Responsable ka para matiyak na ang Personal Data na iyong ibinibigay sa amin ay tumpak, kumpleto, hindi
nakakalito, at palaging napapanahon.

Anumang Personal Data na ibinigay mo ay itatago at panghahawakan ng Kumpanya sa loob ng sampung (10) taon
o hangga't kinakailangan para sa katuparan ng mga layuning itinakda sa Kasulatan ng Pahintulot na ito, o hangga’t
kinakailangan upang masunod ang anumang legal, regulatory at/o accounting requirements.

Ayun sa RA 10173, ang Personal Data ay maaari mong ma-access, ma-itama, mabago, o ma-iwalat kapag iyong
hiniling. Samakatuwid, ang pag-access at paghiling para sa pagwawasto ng iyong Personal Data, at/o upang limitahan
ang pagproseso ng iyong Personal Data, at/o paghiling na iwalat o sirain ang iyong Personal Data, o ang pagbibigay ng
anumang mga katanungan o reklamo tungkol sa iyong Personal Data, ay magagawa sa pamamagitan ng pakikipag-
ugnay sa amin sa address ng aming opisina na kasalukuyang nasa:

Alinsunod sa RA 10173, may karapatan ang Kumpanya na tanggihan ang iyong kahilingan para sa pag-access o
pagwawasto sa iyong Personal Data; ang pagtanggi na sumunod sa iyong kahilingan ay sasamahan ng dahilan para sa
pagtanggi.

Dahil sa mga naunang nabanggit at matapos na maipaliwanag ang lahat ng ito sa iyo, mangyaring kumpirmahin
ang iyong pahintulot sa pamamagitan ng pag-pirma sa kasulatan na ito. Ang iyong pirma ay nangangahulugan na
iyong nabasa at naunawaan ang mga nilalaman ng Kasulatan ng Pahintulot para sa Pagproseso ng Personal na Data na ito
at ibinibigay mo ang iyong pahintulot sa pagkolekta at pagproseso ng iyong Personal Data at mga Sensitibong Personal
Data ayon sa nakatakda sa ilalim ng Philippine Data Privacy Act of 2012 (RA 10173).

The Company reserve the right to amend this Consent Form at any time and will ensure that a notice of such
amendments will be made available to you through any other mode we view appropriate and suitable.

You are responsible for ensuring that the Personal Data you provide us is accurate, complete, and not misleading and
that such Personal Data is kept up to date.

Any Personal Data supplied by you shall be kept and retained by the Company as long as necessary or for a maximum
of ten (10) years, for the fulfillment of the purposes set out on this Consent Form or as is required to satisfy any legal,
regulatory and/or accounting requirements.

RA 10173 requires that Personal Data may be accessed, corrected, modified, or destroyed upon your request. Hence,
access and request for correction of your Personal Data, and/or to limit the processing of your Personal Data, and/or to
request to destroy your Personal Data, or make any enquiries or complaints in respect of your Personal Data, by contacting

Pursuant to the RA 10173, the Company has the right to refuse to comply with your request for access or correction to
your Personal Data, such refusal to comply with your request will be accompanied by a reason for the refusal.

In view of the foregoing and after everything has been explained to you, please confirm your consent by signing this
form. Your signature shall mean that you have read and understood the contents of this Data Privacy Consent Form and
consent to the collection and processing of your Personal Data and Sensitive Personal Data as defined under Philippine

KASULATAN NG PAHINTULOT PARA SA PAGPROSESO NG PERSONAL NA DATA

Sa pagsunod sa Philippine Data Privacy Act ng 2012 (RA 10173), ang iyong pahintulot ay maaaring kakailanganin ng
LSERV Corporation [pagkatapos nito ay tinatawag na "KUMPANYA"] upang maproseso ang mga personal na data
kasama ang mga impormasyon kung saan maaring makuha ang iyong pagkakakilanlan at ang mga sensitibong personal
na impormasyon [sama-sama na tinatawag na "Impormasyon"] na kasalukuyang pinanghahawakan ng Kumpanya. Sa
pamamagitan ng iyong pagpirma sa kasulatan na ito, ikaw ay nagbibigay ng pahintulot sa Kumpanya upang maproseso
ang iyong "Personal Data" para sa mga layunin ng pagtatrabaho at ng pagtatalaga sa mga kliyente.

Maaaring kasama sa Personal Data ang mga impormasyon patungkol sa iyong pangalan, address, mga detalye
ng komunikasyon, resume, mga detalye ng kapanganakan, edukasyon, kasaysayan ng pagtatrabaho, at mga resulta ng
pagiimbistiga ng iyong nakaraang mga trabaho (kabilang na ang pag-uusisa sa iyong background, kredito at references),
mga kopya at iba pang detalye ng mga dokumento ng pagkakakilanlan, katayuang marital, lahi o etnikong pinagmulan,
pisikal o mental na kalusugan, o kondisyong medikal, paggawa o di-umano'y paggawa ng anumang pagkakasala o
paglilitis para sa anumang pagkakasala na ginawa o di-umano'y nagawa kasama na ang dating maling pagkilos, ang
kinahinatnan ng mga naturang paglilitis o ang sentensiya ng anumang hukuman sa mga paglilitis, at mga impormasyong
nakuha sa mga sistema ng seguridad (kabilang na ang mga recording ng boses, video, Closed Circuit Television (CCTV) at
iba pang talaang pang-seguridad), kabilang rin ang mga personal na detalyeng pampinansiyal at mga detalye ng iyong
social security, ang mga personal na detalyeng pampinansiyal ng iyong mga kamag-anak, at ang iyong mga detalye ng
komunikasyon sa pagkakataon ng emergency.

Ang anumang Personal Data kabilang ang karagdagang impormasyon na maaari mong maibigay sa Kumpanya
ay maaaring makolekta, matala, ma-organisa, maimbak, ma-update, mabago, makuha, maging basehan, magamit, at
mapagsama-sama para sa mga layunin ng kontrata ng serbisyo o iba pang kontrata na maaaring pinasok sa pagitan mo
at ng Kumpanya para sa layunin ng pagtatrabaho.

Bilang karagdagan sa nabanggit na layunin, ang iyong Personal Data at iba pang personal na data na maaaring
pagkatapos nito ay nakolekta kasama na ang mga sensitibong personal na impormasyon ay maaaring makolekta,
matala, ma-organisa, maimbak, ma-update, mabago, makuha, maging basehan, magamit, at mapagsama-sama ng
Kumpanya para sa mga sumusunod na dahilan:

a) pagpoproseso ng mga aplikasyon sa pagtatrabaho kabilang ang mga pag-uusisa bago makuha sa trabaho
(pre-employment checks);

b) pagtukoy at pagsuri sa mga suweldo, insentibo, bonus at iba pang mga benepisyo;

c) pagsasaalang-alang para sa promosyon, pag-unlad sa karera, pagsasanay, secondment o paglilipat,

pagsubaybay sa pagganap ng trabaho, pangangasiwa sa kalusugan at kaligtasan, at seguridad at kontrol sa
pag-access;

d) pagsubaybay ng pagsunod sa aming mga panloob na alituntunin at patakaran, at pagsunod sa aming mga
alituntunin sa pag-uugali ng negosyo;

e) pagsunod sa mga kinakailangan sa pagsunod at pagsisiwalat ng anuman at lahat ng mga kagawaran ng

Gobyerno at/o mga quasi-governmental departments at/o mga ahensya, regulatory at/o statutory bodies;

f) upang ipatupad ang aming mga karapatan sa ilalim ng mga naaangkop na batas upang ipagtanggol ang
aming mga karapatan sa ilalim ng mga batas kabilang ang ngunit hindi limitado sa anumang aksyong
pandisiplina o mga aksyon na may kaugnayan sa pagwawakas ng pagtatrabaho; at

g) mga layuning may kaugnayan dito.

Ang mga empleyado at mga posibleng maging mga empleyado ay dapat magkaroon ng kamalayan na ang hindi
pagbigay ng mga naturang impormasyon ay maaring magresulta sa di pagproseso namin ng mga aplikasyon sa
pagtatrabaho, o ito ay maaaring makaapekto sa aming kakayahang isagawa ang aming mga obligasyon sa ilalim ng
anumang potensyal o umiiral na kontrata ng pagtatrabaho at/o sa iyong kakayahang matamasa ang mga benepisyo ng
iyong pagtatrabaho.

Ang Personal Data na hawak ng Kumpanya na may kaugnayan sa mga empleyado at sa mga posibleng maging
mga empleyado ay mananatiling kompidensyal ngunit maaaring magbigay ang Kumpanya ng naturang impormasyon sa
mga sumusunod na partido, para sa mga layuning itinakda sa itaas:
