Notice of Intent to Vacate

I/We the resident(s) hereby provide notice that all residents will be terminating our tenancy of Apartment _________________ at 2323
Sherman Ave. NW, Washington, DC 20001. I/We intent to vacate the apartment on ________________________________________.

We understand the following:

a. This notice does not excuse us from any obligations under the lease agreement.
b. After providing notice, should we subsequently wish to cancel or extend our notice to a later date, we must contact the
Management Office immediately in writing. If our apartment has already been re-rented, we understand that it will not be possible
for Management to grant our request.
c. Should we fail to deliver possession of our apartment on or before the agreed upon date, we will be responsible for additional
rent and fees, and damages incurred by Management.
d. If we have provided a 60-day notice which expires on the date of expiration of the lease term and are moving out during the last
month of the term, we will be responsible for rent and other charges through the lease end date.
e. If we are terminating the lease and moving out prior to the last month of the lease term, such termination constitutes an early
termination under the terms of the lease, and unless we accept a buy-out option (described below), we will be responsible for
the payment of rent through the remainder of the lease or until the apartment is reoccupied, whichever comes first.
f. If we elect to pay the buy-out and any other charges as set forth below and we pay those charges prior to moving, we will no
longer be responsible for rent on the apartment after our move-out date. We will, however, be responsible for any damages
beyond normal wear and tear to the apartment and any outstanding balances.
g. Any refunds and return of security deposit will be issued in the names of all leaseholders.
h. The forwarding address information must be included to ensure timely delivery of any refund and/or return of security deposit.

1. Buy-Out Option: We are terminating our lease early and elect to provide 60 days’ notice, as well as to pay the buy-out equal to
one (1) month’s rent. We understand that if we fail to pay this amount before the day we move out, this option will no longer be
available, and we will be charged rent until the apartment is reoccupied. Initials: ________________
2. Rent Responsibility Option: We are terminating our lease early and do not wish to accept/pay a buy-out as set forth above. We
understand that in doing so, we will remain responsible for rent on the apartment until the earlier of (a) the end of the lease term,
or (b) the date a new resident moves into the apartment. After we move out, we will be charged rent on the first of each month
following our move-out date. Once our apartment is reoccupied, we will receive a credit for the number of days covered by the
new resident’s lease. Although Management will try to re-rent our apartment, we understand that there is no guarantee as to
when a new resident will move in. Even if our apartment is pre-leased, the applicant may cancel. Initials: ________________

In an effort to make your move-out even easier, we have designed a cost effective, stress-free cleaning program, which
covers the essential cleaning required in your lease. It allows you to spend more time packing your things, rather than
cleaning the kitchen!

It’s simple! Just indicate your election for the service on the next page and return to office with a check payable to
“907 Barry Place NW (DC) Owner, LLC”.

For $200.00, the cleaning service will include the following:

Kitchen
• Clean the refrigerator, shelves and crisper, under crisper and under food guard.
• Clean the cupboards, kitchens sink, under sink, countertops and faucet fixtures.
• Clean the microwave and stove, under burners, controls, burner rings, drip pans and oven.
• Clean and sanitize dishwasher. Mop and clean floor.
Living Room and Dining Room
• Baseboards cleaned, finger prints or other marks cleaned off switches and walls.
• Window sills cleaned, windows washed, blinds dusted and cleaned. Dust and clean light fixtures.
• Mop and clean floor.
Balcony (if applicable)
• Balcony glass door washed, screen cleaned, outdoor surface swept. Dust and clean outdoor light.
Bathroom(s)
• Clean bathroom tub, grout, caulking, toilet and tank, sink, vanity and medicine cabinet.
• Clean and dust fixtures. Mop and clean floor.
Bedroom(s)
• Vacuum carpet in bedroom and closet.
• Baseboards cleaned, finger prints or other marks cleaned off switches and wall.
• Window sills cleaned, windows washed, blinds dusted and cleaned. Dust and clean light fixtures.

On your moving day, just turn in your keys and we’ll take care of the rest! Please notify us prior to your move-out date, if
you wish to take advantage of this program. The cleaning will take place after you move-out.
