in the Permit to Work System

• controlled documents
• Used to identify hazards, co-ordinate,
communicate, control and record work Identify Hazards

• Required for most work activities and they are Co-ordinate
issued from the Control Room.

Work Permits
• There are three types of Work Permit
used within the Dukhan Operation.
• The Cold Work Permit
• The Hot Work Permit
• The Radiography Permit.

• Each Work Permit has three Copies,

known as the Original, Copy One and
Copy Two.
• The original copy goes to the worksite
with the Worksite Supervisor, Copy One
stays in the Control Room and Copy Two
is a spare copy used where required.
• Cold Work and Radiography Permits are
valid for a maximum of fourteen
consecutive days from the date of issue.
• Let’s now have a look at these Work
Permits in more detail.

Cold Work Permit

• Has a blue border with a ‘C’ in the
corner
• Used to control work such as
pressure testing or bolting and
unbolting flanges on pipelines.

Hot Work Permit

• Has a red border with an ‘H’ in the corner.
• Used to control work where heat off sparks
may be generated. Can also be used for work
which, involves naked flames, such as
welding, burning or grinding.

Radiography Permit
• Has a yellow border with an ‘R’
in the corner and a radiation sign
watermark.
• Used where the work activity
involves radioactive sources

• There is also ‘Transfer of Responsibility’

box to allow the work to continue under the
control of a subsequent Permit Applicant
and Worksite Supervisor.

For confined Space Entry an Entry Log is mandatory to keep track of all
personnel. The entry log is used to record all personnel entering and
exiting the confined space
