BASIC REQUIREMENT

For Final Transcript

i- Written Application with the name of Controller of Examination


ii- Student CNIC Copy
iii- Copy of Overall Department Clearance form

For Degree

i- Written Application with the name of Controller of Examination


ii- Final Transcript Copy
iii- Student CNIC Copy
iv- Clearance Certificate (Issued By Finance Dept)

IN CASE STUDENT HAS SENT SOMEONE ELSE FOR COLLECTION

Note:-
Only a person having blood relation with the student is authorized

i- Copies of Required documents (As mentioned above)


ii- Authority Letter (As per approved format by Students Services Department )
iii- CNIC copy of Student
iv- CNIC copy of Authorized Person with Original

DOCUMENTS REQUIRED FOR THE ISSUANCE OF REVISED/DUPLICATE


DEGREE/TRANSCRIPT
The following documents are required for the preparation of Revised/duplicate
degree/Transcript/diploma.

Check list for Revised Degree

1- Application from student on prescribed form


2- Copy of old degree/Transcript
3- Copy of CNIC
4- Copy of SSC(old +revised)
5- Fee Receipt for Duplicate/Revised Degree
6- Reason /justification for revised degree.

Check list for Duplicate Degree

1- Application from student on prescribed form


2- Copy of old Degree
3- Affidavit on judicial paper worth Rs.20/- given reason for issuance of duplicate
Degree/diploma/certificate. (Reg# & Old degree serial # and degree title must be
mentioned)
4- 01 passport size photograph.
5- Copy of National Identity Card.
6- A copy of press cutting of a National News paper notifying loss of
Degree/diploma/certificate. (Reg# and Degree Serial No should be mentioned in ad)
7- Reason/Justification for Duplicate Degree.
8- In case of lost/theft of the degree/diploma/certificate, original copy (duly stamped)
of FIR from the respective Police Station.

Check list for Duplicate Transcript

1- Application from student on prescribed form


2- Copy of old Transcript
3- Copy of National Identity Card.
4- Reason/Justification for Duplicate Transcript.

Note:
The above mentioned documents are checked / verified and attached with the application form.
