AFFIDAVIT OF LOSS

I, _____, of legal age, Filipino, married and a resident of _____, Pampanga, after
having been duly sworn to in accordance with law, do hereby depose and state:

1. I am one of the children of the late _____, who passed away on __________
2008;

2. My mother, _____, along with her brothers and sisters, namely: xxxxx, xxxxx,
xxxxx, all surnamed delos Reyes, are the registered owners of two parcels of land
situated at
______ and ______ Pampanga, covered and embraced under OCT Nos. _____ &
______ of the Registry of Deeds of Pampanga;

3. During our mother’s life time, she was entrusted with the safe keeping of the
owner’s duplicate copies of the subject land titles;

4. However, after her passing on ______, we sorted her possessions, only to


discover that the owner’s duplicate copies of OCT Nos. _____ are missing;

5. With the consent and knowledge of our relatives, I caused the filing of a
petition for lost title, which was heard before the Regional Trial Court, ______ of
Pampanga, docketed as _______;

6. RTC Br. granted our petition and ordered the Registry of Deeds of Pampanga
to issue us new owner’s duplicate copies of OCT Nos. _____ in lieu of the lost ones, a
copy of the Resolution dated March is hereto attached ready for reference;

7. Upon being issued a new owner’s duplicate copies of OCT Nos. _____, I kept
the same in our office at _____ Porac, Pampanga;

8. But recently, when I attempted to retrieve these from my office files, I could not
find the subject land titles, in spite of diligent efforts;

9. I do not recall of having these surrendered to any person for any reason nor
were the same confiscated by authorities for the commission of a wrong;

10. I am therefore executing this affidavit to attests to the foregoing statements


and for the purpose of declaring owner’s duplicate copies of OCT No. _____ LOST for
all intents and purposes;
