Good day!

As part of the Department’s recruitment and selection process, we have attached herewith a Consent
Form for your review and signature. In addition, we kindly request that you also fill out the List of
Employers for background check purposes.

HRMO wishes to note that you ​may withhold your permission and that in such a case, no background
check will be done, and your application for employment will not be processed further.

Please send the signed Consent Form (in PDF) and the accomplished List of Employers (in PDF) on
or before (a day from the date of email).

purpose of confirming the information contained on my application and/or obtaining other
information which may be substantial to my qualifications for employment.

I acknowledge that I may withhold my permission and that in such a case, no background check
will be done, and my application for employment will not be processed further.

I further acknowledge that the scanned or photocopy of this document shall be considered valid
and accepted with the same authority as the original.
