ii. Student’s Year of Study
iii. Name of organization where field attachment was undertaken
iv. Field attachment period
v. Name of student and Registration Number
vi. Name of the Supervisor

This should provide a summary of objectives of the field attachment, duties
performed, benefits derived from the field attachment as well as the major
challenges, conclusions andrecommendations.
v. LIST OF FIGURES/PICTURES
All figures/pictures must be captioned at the bottom, with figure numbers shown
against each caption.Example. Figure 1- organization structure of Ilala district
council.
N:B: Each Sub-title shown above (ii-vii) should be written in capital letters and at the
centre of respective page. Each should be written on a separate page.
