The cover page should include the following things in the given order:
i. Name of College
ii. Student’s Year of Study
iii. Name of organization where field attachment was undertaken
iv. Field attachment period
v. Name of student and Registration Number
vi. Name of the Supervisor

This should provide a summary of objectives of the field attachment, duties
performed, benefits derived from the field attachment as well as the major
challenges, conclusions andrecommendations.

iv. TABLE OF CONTENTS


A table of contents should show the contents of the report and the page numbers for
easy reading and referencing.

v. LIST OF FIGURES/PICTURES
All figures/pictures must be captioned at the bottom, with figure numbers shown
against each caption.Example. Figure 1- organization structure of Ilala district
council.

vi. LIST OF TABLES


All tables must be given headers /titles and with table number shown against each
table header as well.

vii. LIST OF ACRONYMS AND ABBREVIATIONS


All acronyms or abbreviations used in the report should be included in this section,
with their full meanings.

N:B: Each Sub-title shown above (ii-vii) should be written in capital letters and at the
centre of respective page. Each should be written on a separate page.

INTRODUCTION

1.1. Background of the field attachment program.


1.1.1. Explain what is field attachment?
1.1.2. State Objectives of the field attachment (General and specific
objectives)
1.2. Background of the organization attached to.
1.2.1. Historical background of the organization where a student was
attached.
1.2.2. Mission, vision and objectives of the organization
1.3. Major activities of department that a student was attached to
1.4. Structure of the organizationwhere a student was attached to with emphasis
on the department where student was attached.

EXPERIENCES
2.1. Explain the duties and responsibilities you were given/assigned in relation to
your
Objectives (make sure you match the assigned duties with your field of study)
2.2. Explain the new knowledge and skills gained from the duties and
responsibilities

2.3. Level of accomplishment of the duties/responsibilities assigned (rate the
status/level of
your perfoamnce on the assigned duties in relation to the knowledge and skills
provided at the College)

CHAPTER THREE

CONCLUSION AND RECOMMENDATIONS


3.1. Conclusion
Remember to include the major strengths and weaknesses of the field attachment
program.
3.2. Recommendations
Include recommendations for strengthening the field attachment program as part of
the teaching curriculum at TPSC; and for improvement of the services delivery at the
organization you were attached to.

4. REFERENCES
Indicate all relevant reference materials used during the field attachment and/or
report writing such as textbooks, personal communications, internet material, codes
of practice and guidelines.

The reference materials must be quoted as follows:

Name of author(s) or editor(s), Year of publication in bracket, Title of the book,


Edition, Name of publisher and Place of publication.

Example: Armstrong M (2005), Human Resource Management Practice, Kogan Page

Finally, submit final field attachment report to your supervisor ready for marking.

FINAL REPORT REQUIRMENTS

a) It should be spiral binded


b) The report cover should be green for certificate students and Orange for
diploma students
c) The font Style should be Times New Roman.
d) Font size for the cover page should be 14 and 12 for the rest of the report.
e) Line spacing should be 1.5.
f) Page numbers should be inserted on each page of the report (Use roman
numbers for preliminary pages i.e i, ii, iii........ and numeric for the rest of the
riport i.e 1, 2, 3,.......).
