GUIDELINE / CHECKLIST FORMAT FOR PRACTICAL ATTACHMENT

I. SAMPLE STUDENT EVALUATION FORM

This is a quick instrument for evaluating your practical attachment situation. Your views are
urgently required for preparation of a report to the Instructor or to the department. This
evaluation form should be completed as soon as the practical/field attachment ends.

A. SPECIFIC GUIDELINES
1. Identify prior interests in relation to your field of study?
2. Select the organization tied with your prior interests in your locality?
3. Follow up on assignments.
4. Communicate the organization that you selected in relation to
(a) Where are you from ( Introducing yourself and subject matter)
(b) Provide information regarding your Practical work training ( the objective
of practical training, the number of days it needed, the need of replying
letter to Economics department in Injibara University , a general
information on your practical work)
5. If you don’t get permission in your selected organization, try to communicate another
organization related to your field?
6. You should come up with confirmation letter from the organization you tied with the
practical attached. Ask the organization to write the reply letter to Injibara University,
Economics department about;
a. Is the trainee doing practical work properly or not? Yes____ or No
b. How Long the trainee takes practical work in collaboration?__________ no.
days
c. How was the ability of trainee in the work? (Very Good, Good, Medium,
Low and Poor)

7. Prepare the following two kinds of reports;


A. General process of your practical attachment work.
B. The organization’s work efficiency.
8. Get ready and be able to present the report.
9. You should submit your reports in hard and soft copy to the assigned Instructor.

C. SAMPLE FIELD ATTACHMENT REPORT FORMAT AND GUIDE


The field attachment report should be well written in good English (Times new roman, 12
font size, 1.5 line and paragraph spacing, white highlighting, black font colour e.t.c ) and
should at least contain the following contents.

1. COVER PAGE
This should show the following in the given order: Name of the college; name of department,
field attachment period (e.g. July – September 2022); Name of student, Student’s
Registration Number; Trainer’s Signature.
2. ACKNOWLEDGEMENTS
The student should acknowledge all those who assisted in contributing to the success of
the field attachment and report writing.
3. SUMMARY/ABSTRACT
This should provide a summary of the most important practical work the student learnt
as well as the major challenges, conclusions and recommendations.
4. TABLE of CONTENTS
A table of contents should be generated which shows the contents of the report and the page
numbers for easy reading and referencing.
5. LIST of FIGURES if ANY…
All figures must be captioned at the bottom, with figure numbers shown against each
caption. The pages where particular figures are found should be indicated for easy reading
and referencing. Figures should be preferably numbered according to the chapter numbers.
6. LIST of TABLES if ANY…
All tables must be given headers and with table number shown against each table header as
well. The pages where particular figures are found should be indicated for easy reading and
referencing. Tables should be preferably numbered according to the chapter numbers.
7. LIST of ACRONYMS/ABBREVIATIONS if ANY…
All acronyms or abbreviations used in the report should be included in this section, with
their full meanings.
8. INTRODUCTION
Background of the field attachment program (refer to the brief guiding information
handout)
Objectives of the field attachment
Background of the district attached to
Structure and organization of the district with emphasis on the department where
student is deployed (Is the company small, medium or large size firm?; In which
sector the firm produce (agriculture, industry, service)?; Does the firm have strategic
and operational plans?).
Major activities and focus of the department
9. EXPERIENCES
Duties and responsibilities
New knowledge and skills gained in each of the duties and responsibilities assigned.
Level of accomplishment of the duties/responsibilities assigned (may indicate your
contribution in terms of new knowledge and skills provided)
Relationship with other staff
Things enjoyed most and least. Why?
Problem experienced and how handled
Major benefits derived from field attachment program
10. CONCLUSION and RECOMMENDATIONS
Conclusion
Remember to include the major strengths and weaknesses of the field attachment program.
Recommendations
Include recommendations for strengthening the field attachment program as part of the
teaching curriculum.

11. REFERENCES
All relevant citations such as textbooks, personal communications, internet material, codes of
practice and guidelines which student uses during the field attachment and/or report writing
must be quoted using the Harvard style of referencing.
