LIST OF REQUIRED DOCUMENTS FOR PF & ESIC REGISTERATION

 Name of the Company

 Date of the setup of the Organisation

 Scanned copy of the company’s PAN (Proprietor’s,in case of proprietorship concern)

 Scanned copy of the licenses available in the name of the company (Like GST)

 Scanned copy of cheque of company’s bank account

 Proof of premises(Address) : Agreement copy / leave – license agreement copy ( electricity bill),
(Original in PDF )

 Memorandum of articles, Incorporation certificate & partnership deed.

 If Partnership firm or Private Ltd all partners or Directors Individual PAN card and DIN No.

 Residence Proof of Directors, Partners & Proprietor (Electricity bill)

 List of partners / director with name, PAN, Aadhar, email id, mobile no & address.

 Email address, Mobile Number of Proprietor/Directors/Partners

 No. of employees with date of joining

 Employee strength since beginning.

 Nature of work.

 Bank details (Name, address & account no.) and last three month bank statement.

 Cancelled cheque for each registration.*( original PDF copy of Cheque. )

 Shop & Est. License./ Factory Licence. *( original PDF copy of Shop Act Licence.)

 Consent letter of employees. *( original PDF copy of Consent letter for voluntary coverege. )

 Specimen signature of authorized person ( Resolution.) ( Original PDF copy )

 Xerox copy of master & salary register of coverage month.

 CST / VAT/GST / P.Tax Certificate ( Original PDF Copy)

 Branch details like name of branches, address, details of employees (branch wise)

 Balance sheet / P & L accounts. ( in case of limited company Annual report since beginning. )

 Business agreement, Work order (2 nos.)

 Pasara License. ( If Security Agency )

 Digital Siganture of Employer with password.( Class 2 )


 Authority letter for us to represent your organization in all registration department.

 PAN no, Aadhar card no, and Bank details with IFSC code of employees Mobile No

 If the place of Business is on rented requird SHOP OWNERS DETAILS NAME, FATHERS NAME,
GENDER, DOB, PAN CARD, MOBILE NO, EMAIL ID, DATE FROM OWNERSHIP
POSITION,RESIDENTIAL ADDRESS
