SCHOOL PROFILE
Mambagaton National High School is situated in Barangay Mambagaton, Himamaylan City, Negros Occidental, located almost for
(4) kms (North-East) of Himamaylan City Hall. It was first established in 2004 and operated as Annexed School of Aguisan NHS
and used the old Mambagaton Barangay Hall as temporary classroom. On March 12, 2009, a 2-hectare school site at Sitio Kasoy,
Barangay Mambagaton, was donated and was formally turned-over by the City Government Recognition (R.O. VI) No. 032, s.
2009.
Mambagaton National High School was then converted to Mambagaton Farm School in the year 2021.
The school is offering Junior High School (Grade 7-Grade 10) and Senior High School (Grade 11- Grade 12). The school have 493
learners officially enrolled for School Year 2021-2022 in both Junior and Senior High School with 14 JHS and 3 SHS Teachers.
We had ten (10) classrooms, in which five (5) are make-shift classrooms and four (4) comfort rooms. Every learner had its own seat
for learning and clean toilet for proper sanitation.

GEOGRAPHY
Mambagaton National High School is situated in Barangay Mambagaton, Himamaylan City, Negros Occidental, located almost for

Mambagaton Farm School was established in 2019. The school is situated in Sito Kasoy, Barangay Mambagaton, Himamaylan
City. Farming is the main source of livelihood in the community.
COVID-19
New normal caused by the COVID-19 pandemic brought a lot of changes and challenges in schools where face-to-face
interaction is vital. Since its outbreak in late December 2019, the pandemic has created havoc across the world and like any critical

sector, education has been hit hard. Varied modalities have been assessed and applied as the country thrive to shift from the
traditional way of learning to new learning modalities. Through the various adjustments, the school still aims to produce quality
graduates through a well-managed school and well-implemented curriculum that will provide worthwhile experiences to learners. As
the Department of Education (DepEd) continues with its commitment to ensure learning continuity and to protect the safety, health,
and well-being of learners amid the COVID-19 pandemic, proposed the implementation of Pilot face-to-face classes in minimal risk
area that will be implemented in January 2022. In line with this, Mambagaton Farm School was chosen as one of the school
participants in the Philippines. The school is in Brgy. Mambagaton, Himamaylan City, which belongs to the low-risk barangays in
Himamaylan City. Necessary actions and precautionary measures are taken by the school community to ensure parent’s consent
from the total number of 79 Grade 7 students who will participate in the proposed Pilot implementation of Limited Face to Face and
who are residing in Brgy. Mambagaton, Himamaylan City.
Also, teaching staff of Mambagaton Farm School are all vaccinated prior to the date of implementation of the program. The
school craft its CONTINGENCY PLAN through the collaborative effort of all the faculty and staff, in coordination under the Inter
Agency Task for of Himamaylan City, with the support of the Schools Division of Himamaylan City, shall serve as guide of the
community members during the implementation of Limited Face to Face Program amid COVID-19 Pandemic.
In line with this, Mambagaton Farm School will disseminate the Contingency Plan to the school community and the local
Government Unit for them to be aware of the critical actions needed as a response to any COVID-19 outbreak. The objective of this
plan is to minimize the impact of COVID-19 to the school community and to ensure continuity of learning if a member of the school
community becomes a close contact, suspect, probable, or confirmed COVID-19 case if a school lockdown is needed due to the
resurgence of COVID-19 within or outside the school.

Contingency Plan for the implementation of the pilot limited Face-to-face Program which will start on August 2022, and which
covers the total number of 79 participating Grade 7 students. This document was developed in collaboration with Disaster Risk
Reduction and Management Service, Inter Agency Task Force, Local health authorities, Local Government Unit, school health
coordinator with all affected agencies in the Division of Himamaylan City. This Contingency Plan aims to:
1. Adopt the School Contingency Plan Manual of the implementation of the limited face to face classes.
2. Establish the coordination and communication mechanisms among the National, Regional, Division, District offices
3. Issue memorandum on critical actions to be done by the school to ensure a timely school-based response to any case
scenarios that may affect school operations.
4. Ensure continuity of learnings if a member of the school community becomes a close contact, suspect, probable, or
confirmed COVID-19 case or if a school lock down is needed due to a resurgence of Covid-19 cases within or outside the
school.
5. Determine the preparedness, prevention, and mitigation measures to minimize the impact of the Covid-19 pandemic.
6. Maintain safety and well-being of the learners, teachers and other personnel that will be participating in the
implementation of the pilot limited face to face.

a. Who: name of individual gender, age, grade level or occupation, the section included in or handling
b. When: date, if in school (i) time of entry to school, (ii) time of isolation in the clinic if at home (i) time of report
c. Where: school name, school address, the home address of the individual (if at home during the time of the report)
d. What: symptoms experienced by the individual (if any)
e. How: details on where or how the individual was infected, or who is the confirmed case to whom the individual became a close

community in the necessary actions in case there will be a need to transition to a distance learning modality.

Contact the identified health authority (e.g. barangay health station, rural health unit) to cooperate regarding the appropriate
measures such as tracing and quarantine, or referral to a hospital if needed.
b. Contact other school personnel (from the different set of classes per week) in case there will be a need to shift to a distance
learning modality. A teacher must also be informed and guided if he/she shall be a substitute teacher in case a fellow teacher
becomes unable to physically report to school for a long period due to being infected with COVID-19 or other acceptable reasons.
c. Contact the family of the individual who showed symptoms or identified to be a close contact of a confirmed case of COVID-19.
The family must also be informed regarding the actions that will be undertaken by the school, as well as the protocols that must be
observed at home as advised by health authorities.
d. Contact the families of individuals from the same class as the person who showed symptoms of COVID-19 and inform them of
the protocols that must be observed, as well as the possible shift to distance learning modality.
e. Contact families from other classes from the set of classes in the same week as the person who showed symptoms of COVID-
19, as well as those from the set of classes in the other week to inform them of the current school situation.
2. Clean and disinfect school buildings, classrooms and especially water and sanitation facilities at least once a day, particularly
surfaces that are touched by many people (railings, lunch tables, sports equipment, door and window handles, toys, teaching and

remained asymptomatic throughout their infection.
For mild to moderate COVID-19 confirmed cases: Ten (10)-day isolation has passed from the
onset of the first symptom, respiratory symptoms have improved (cough, shortness of
breath), AND have been afebrile for at least 24 hours without the use of antipyretic
medications.
For severe and critical COVID-19 confirmed cases: Twenty-one (21)-day isolation has
passed from the onset of the first symptom, respiratory symptoms have improved (cough,
shortness of breath), AND have been afebrile for at least 24 hours without the use of
antipyretic medications.
For immunocompromised, do RT-PCR testing on the 10th day. If RT-PCR test results turn
out positive, refer to Infectious Disease Specialist. If RT-PCR test results turn out negative,
discharge from isolation.
Medical certification or repeat testing is not necessary for the safe return to the school of
immunocompetent individuals, provided that a licensed medical doctor certifies or clears the
patient. He/she shall be advised to isolate at home or in an isolation facility until the COVID-
19 test results have been released.

cleaning and maintenance routine of the school, and those who will need to perform administrative duties whenever necessary. The
school shall ensure that they will be properly compensated for physically reporting to school during the school lockdown.
5. The guidance office shall remain operational to ensure that psychosocial support and services shall be made available for all the
members of the school community. In the absence of a guidance office, affected learners and school personnel shall be provided
with Psychological First Aid (PFA) by trained teaching and non-teaching personnel in schools. They shall also be assisted for referral
to the counselling services provided by the Schools Division Office, if further interventions are needed.
6. The school shall provide the appropriate support needed by the non-teaching personnel while they are unable to physically report
to school on a daily basis.
After the school lockdown, the school shall seek the consent of parents/guardians through a written form for the participation of
their children in the resumption of face-to-face classes. 8. For the first five school days, the first hour shall be devoted to discussion
facilitation of modules related to mental health, facilitated by their respective classroom advisers or designated teachers.

The following personal strategies are behaviors that school staff and students can use to protect themselves and others around
them.
Masks and respiratory hygiene
Wear a mask when physical distancing of 1 meter is not possible and especially when individuals are indoors with poor ventilation
(such as a crowded classroom). Consistent and correct mask use protects both people who wear them and the people around
them.
Cover coughs and sneezes using an elbow or a tissue when not wearing a mask. Dispose of the tissue and clean hands
immediately either with soap and water or a solution containing at least 60% alcohol. Students should be frequently reminded not to
touch their eyes, nose, or mouth or masks.
Stay home when sick, or after close contact with someone who is sick. If a student or staff member is sick, he or she should not
come to school. If COVID-19 is suspected or confirmed, students and staff should self-isolate at home until at least 5 days have
passed since symptoms first appeared, (at least 24 hours have passed since last fever without the use of fever-reducing
medications, and symptoms have improved). Isolation may be extended to 10-20 days depending on severity of illness or until the
national criteria for stopping isolation have been met.
