I, BERNARD F. DOMINGO of legal age, Filipino citizen, a resident address at Blk 78 Barrio San Isidro
Tala Brgy. 188 Dist. 1 Caloocan City, do hereby NAME, APPOINT and CONSTITUTE---- ROXANNE ALLION of
legal age, Filipino citizen, with office address at Imus City Cavite , to be my true and lawful attorney-in-
fact, to act for my stead and perform the following acts;

1. TO TRANSACT AND REPRESENT MY BEHALF AT THE GOVERNMENT OF PALO, LEYTE AND TO


THE HOSPPERTAINING TO THE CERTIFIED TRUE COPY OF MY ANNUAL INCOME TAX RETURN
2021 FOR THE UPDATES OF BOOKS OF ACCOUNT IN MY BRANCH 2 LOCATED AT QUEZON CITY
RDO ____ AND CAUSE ALL TRANSACTIONS RELATED THERETO;

2. TO EXECUTE, DRAW, SIGN, CLAIM AND OBTAIN ANY AND ALL DOCUMENTS NECESSARILY
INCLUDED TO THE AFORE- MENTIONED TRANSACTION;

3. TO DO SUCH OTHER ACT OR THING THAT MAY BE REQUIRED NECESSARY OR INCIDENTAL TO


CARRY-OUT EFFECTIVELY ANY AND ALL OF THE PURPOSES FOR WHICH THIS AUTHORITY IS
HEREBY GIVEN.

HEREBY GIVING and GRANTING unto my representative full power and authority to execute
