LEADING TO INFECTION

January 28, 2022

This is to certify that Mrs. Cheryl T. Tampos, Master Teacher l of Don Ynocencio A.
Del Rosario National High School and an active employee of the said institution.

Due to Covid-19 pandemic, the school adopted Modular Distance Learning to deliver
the needs of the learner. As an adviser to Grade 11- Cookery, her duties are as
follows; she prepares Self Learning Modules in print and distributes them every week
to parents as scheduled; conducts home visitation to check on each student’s
progress and performance; and provides remediation if necessary.

A few days prior to sickness on May 13, 2021, during the distribution of modules to
parents, she was exposed to several individuals unknown to be infected by Covid-
19, despite following precautionary measures like wearing of mask and face shield,
social distancing, and sanitizing. Two days later she found out that she's one of the
close contacts of a positive individual. She then manifested signs and symptoms
such as body malaise and headaches. She sought consultation and a swab test was
done and revealed positive results.
