Dear Participants/Parents,

We, the Research Group 6 of Grade 10 Borello, at Jesus Good Shepherd School, are inviting you to participate
in this research study entitled. The Correlation Between Jesus Good Shepherd School’s Mission and
Environmental Care Conduct of Junior High School Students.
This research endeavor is a requirement for our subject/s English and ESP.

We decided to choose you as our research participant because you are the best fit to participate in the study and
could greatly help us towards its completion. There are no known risks associated with this research. If you
agree, we will ask you questions related to the topic investigated in an informal/formal interview format. Your
name will not be included in the project or any other documents. Additionally, you will have full access to the
completed research paper. You are allowed to withdraw if you feel you need to withdraw because of unforeseen
circumstances. Your participation is purely voluntary and you can withdraw anytime.

If you have questions regarding your rights as a research participant or to report any concerns about the study,
