Main goal
Awareness on safety management system for leaders:
Understand that the money invested in safety is nothing compared to the expenses incurred
in the case of an accident
“If you say safety is expensive, try an accident”
Theme :
Serious Incident (link with pilot and ATC) lead to an emergency evacuation (link with
equipment (doesn’t deploy); training (cabin crew); Maintenance (Maintenance of the slide))
BEGINNING
Story Line
FIRST
- All in a room (plane seats) with a screen (cockpit view of the flight)
- turbulence emergency landing due to bird strike
- deployment of the slides some do not deploy correctly
- evacuation of the players by the slope that are working
- Mannequin are thrown of the plane (simulate one fatality) + cabin crew descend
before passengers (evacuation goes wrong)
“You will now find how a good implementation of a SMS could have avoided this serious
incident to become an accident, you have 10 key messages to find”
Note:
The investigation is really to understand the problem with the emergency evacuation and not with
why there is a emergency landing.

Enigma 1
“Re-do the schedule duty time”:
Two schedules:
- The First One is used by the company and show a bad management of planning
- The second one is empty and need to be fulfill. How to fulfill it? You will have 10
riddles (good & bad ones) only 4 needs to be used. All the riddles will be numerated.
With the 4 numbers corresponding to the riddles, you will be able to open
somethings.
- With the password you can unlock a phone: on the phone you can read a
conversation between the accountable manager and a manager showing that the
accountable manager is over pressuring his operator.
- In the conversation some letters are in CAPITAL. With those letters it forms a word:
the first key message: OVERPRESSURE

Enigma 2
Training program
- On an excel sheet list of all employees with one who has his recurrent training out of
date “safety training for the crew” this person has an id number.
- Post-it on the table “it’s important to remember your ID number”
- ID number used to unlock a computer
- Lead to a timetable of the recurrent safety training that was supposed to be done
two weeks before the accidents but written “CANCEL due to a lack of resources”
- On the timetable there is indication for rooms numerated from 1 to 26 that
correspond to a letter to complete the second word of key messages “training”
(verification with the player if they don’t understand)

- Create the excel sheet list with fake names-fake dates & fake id number

Enigma 3
Audits: only compliance and no safety
- Videos of a cabin crew member making the evacuation signs (4 for example)
- 1 is badly managed, the number of the video is linked with an audit report
- The audit report is only checked with yes/no, but no notes/no details are written.
- On the audit report there is a telephone number to call
- On the phone there is the auditor that explains that he is used to do compliance
audit & not safety audit
- The auditor emphasizes on the word Safety Audit. Safety audit is the word to write
on the “answer page”

be useful for next phases (enigma 4)

Enigma 4
Reporting system
- Piles of paper with a lot of report written “to treat”: there is a report of a person who
worked with an inadapted tool and “cut” the slides because the adapted tool was
missing. This report was sent 1 month before the accident (the number of the plane
is the same that the one with the accident).
Key messages:
Feedback of reporting is necessary to maintain a good reporting culture.

Enigma 5
Resources problem
- The task card of maintenance found in enigma 4 are not done completely.
- Radio: -petit jingle- “The company MecaTronic has had some issues, deficiencies in
critical maintenance task have been reported”
- In the room there is a file with invoices not paid with one related to the task card not
done. (Tools-spare-facilities-human) with half word “res” “ources” “pro” “blem”. It is
the word to write on the “answer page”.

Enigma 6
Safety culture
- On a phone there is a notification: one missed called with a voicemail
- On this voicemail: someone is telling that he will not report because someone was
fired due to the same mistake. But he probably would have reported if the issue of
the company was brought to light
- (There is different normal light and one UV light)
- UV lamp (brought to light) in the room is needed to deliver this message “what is the
notion the company need in terms of culture that allows people to not be punish for
their errors but that doesn’t allow gross negligence”
- Answer is “just culture” and need to be written on the “answer page”

Enigma 7
Negative training & risk analysis
- Risk Analysis of the company, only one risk is identified with
probability : 1 & severity : 1 : it is negative training
- Next to negative training, it is written “check the trainers” as a comment
- On the table there is a file with the different trainers, how they execute a procedure
video of different trainers & the name of the people they recently trained.
- One trainer is not doing the procedure the same way & bypass some steps
- Next to this there is a binder with one interleaf for each trainer with the detailed of
the formation they offer. On the one of the “weak trainers” on each page it is
written one of the words of “know your risks”
- On the “answer sheet” you need to write “know your risks”

Enigma 8
Management of change
- In the room there is a “CGT” flag with next to it an e-mail that is cut
- You must put the pieces together then you will see that on this e-mail that the ATO is
going to recruit 40 employees from another company and the operators had only
known of this one week before the new people arrive
- There is more paper that is needed: on the remaining paper there is some colors.
- The colors are linked with keys
- The key opens a closet, with files insides that explained the importance of always
telling your employees before implementing a change
- The term management of change is never really written but some letters are in bold
and form the word “management of change” word that is needed to write on the
“answer page”

Story Line
You put your file in an urn and then a video appears with a rewind of the flight (first video),
but this time everything goes well in the evacuation.
