TASK RISK ASSESSMENT

What is Risk Assessment?


Process of evaluating the risk(s) arising from a hazard(s), taking into account the adequacy of any
existing controls, and deciding whether or not the risk(s) is acceptable.
Why Risk Assessment?
• Establishes job performance standards.
• Eliminates or minimizes incidents.
• Creates a job training tool for new employee(s).
• Makes safety and health a part of the production process, not an add-on.
• Aids in incident investigations.
• Helps in prioritizing the control measures.
• Helps to decide the acceptability of risk.
Risk Assessment Team:
The outcome / quality of the Risk assessment depend on the composition of team. The
representative from different executing agencies / the person who are performing task should be
involved. Contractor representative should be involved where the task is performed by the
contractor.
How to do a risk assessment
At IEPL we have procedure on Hazard Identification and Risk Assessment (No. IEPL-HSEF-2.325).
The guidance given in the procedure should be followed while carrying risk assessment.
Step 1: Identify a task / activity.
Identify the task and describe the sub task / job steps in the task for risk analysis.
Step 2: Identify the hazards.
In order to identify hazards we need to understand the difference between a ‘hazard’ and ‘risk’. A
hazard is ‘something with the potential to cause harm’ and a risk is ‘the likelihood of that
potential harm being realized’.
Ask your employees, Visualize hazard, in each sub task with respect to……… Physical, Chemical,
Ergonomic, Biological and Environmental hazards.
Walk around the workplace look afresh to assess what could reasonably be expected to cause
harm.
Step 3: Decide who might be harmed and how
Once you have identified a number of hazards you need to understand who might be harmed and
how, such as the person executing the task, other people involved, persons who run a higher risk,
bad health, medication, competency of the person executing the task, training and experience.

Step 4: Evaluate the Initial Risks.


TASK RISK ASSESSMENT
After ‘identifying the hazards’ and ‘deciding who might be harmed and how’ you are then required to
protect the people from harm. The hazards can either be removed completely or the risks controlled
so that the injury is unlikely.
Each hazard of the sub task shall be studied to determine its risk level by evaluation of the risk.
Risk = Probability X Severity
Estimate the probability and severity associated with each risk scenario.
Step 5: Decide on control measures.
Describe the measures that are needed to eliminate the risk or bring it down to acceptable level.
When determining controls, or considering changes to existing controls, consideration shall be given
to reducing the risks according to the following hierarchy:
• Elimination (Eliminate hazard)
• Substitution (Change the method, change of chemicals etc.)
• Engineering Control (Take corrective actions, LOTO, Mechanical ventilation etc.)
• Administrative Control (Training, Procedures, Instructions, Job rotation)
• PPE (Make people wear Personal Protective Equipment’s)
Step 6: Assessment of the Actual Risk:
Determine the effectiveness of the measures taken. The effectiveness of the measures is reflected in
the scores. Any risks identified as “High” or “Extreme” are likely considered unacceptable / Intolerable
and requires additional measures to bring risk level within acceptable limit.

Record the findings, discuss the Risk Assessment findings with the working crew.
Step 7: Review your assessment and update as and when necessary:
The risk assessment shall be reviewed upon Change in the legislation, Organizational change, Any
occupational health issue immerges, Advances in the technology, Control measures found inadequate,
Change in work force, Changes in the proposed corrective and preventive actions.
All Risk Assessments shall be reviewed after three years.
