Tell me about Gabriel's diagnosis and medical history.

So, Gabriel has a very rare form of epilepsy called Lennox-Gastaut


syndrome. It is not curable and so part of what his daily life looks
like is, he has daily seizures and spasms ranging from anywhere
between 10 to like 30 or 40. One of the downfalls is, because he is
having so many seizures and so frequently, he is considered
severely developmentally delayed and he's not verbal. So he
doesn't use like a lot of words and phrases like a typical child
would. But he so, I mean, but overall, he's a pretty happy kid. He
also has CVI, which stands for cortical visual impairment, so often
times if you see him squinting, that's just part of him trying to
focus on things. Yeah, but by no means do his disabilities delay
him from functioning and being a part of everyday life.

Please describe the speech-language pathology services that
Gabriel has received.

And there's just like a funny phrase in the special needs parenting
community where "you don't know until you know." Which
basically is just like, you don't know that you're missing out on
something that could be really beneficial for your child until
somebody tells you. And so we were lucky enough that you had
reached out to us and you were aware of his developmental delays
and kind of introduced us to the idea of AAC therapy. And we went,
we started last summer, it's been a year--started last summer. And
after our first meeting, we were hooked because we discovered
and finally had hope that our kid could communicate with us and
we could communicate with him. We kind of, we didn't know
because nobody told us, and so we didn't realize that we had an
option to be able to communicate with him and like talk to him
without like, without communicating in the same way that you
would an average two-year-old and allowed us to see that that was
a possibility. I feel like since discovering that we really have like

thrived as the family in regards to like communicating with him


and feeling like he understands--yeah, him understanding what it
is--him feeling like we understand what it is that he needs.

Can you tell me more about Gabriel's progress with
communication?

Yeah, so Gabriel uses the Jellybean Button switch, and it's literally
just a little like red--it comes in different colors--but it's just a little
switch and it clicks and you push it. Yeah, aha. And it's attached
with a Tapio to an iPad. And currently that's what he's using as a
talker. So we've got like a talker app on there where he gets to like
talk and express himself. He's been recently learning how to
harvest for language so he has a lot more language than his talker
provides. And so when there is something he is wanting to say
often times he will pick a different option and it's kind of like our
job to figure out what he could be trying to say. But usually it's
pretty close and it makes sense. So we use his talker to figure out
when he's in pain--what it is that is hurting, what part of his body.
We've used his talker to introduce himself to others. He really likes
to talk about himself and his family. We also use his talker to
decide what he wants to play. And he uses it with all family
members. So, we've been lucky enough to get to bring him to,
bring him to sessions with other family members--so grandmas,
grandpas, aunties, uncles. And they've all been able to see how it
works. And they very kindly and just amazingly have just integrated
it into their daily lives as well. So he's getting the consistency to
communicate with people, and so that's why it's working.

Describe Gabriel's needs that occupational therapy has addressed
in the past.

So before we, before we had talked to you, he was using a lot of a


variety of seating options that just weren't really good for his
posture, and we didn't know that because nobody told us. And so
he has, you know, like a pelvic tilt and his thumbs are frequently
inward, which is not normal and it's not ideal for like utilizing, you
know, devices and interacting with the world. And so we realized
after seeing you that like you kind of like have made us aware and

given us the accessibility to mobility devices such as the chair that


he's currently sitting in, which is specifically measured to him. We
use the Zippie Voyage and it comes with a cool little tray that his
button switches sit on. We also have the Manatee bath chair, and
that was life changing for our family as a bath chair because prior
to that we were using like an inflatable bath and and it was a
struggle to keep him in it. So it really has allowed bath time to be
more fun. The chair allows us to get places, like we've been able to
actually go on vacations and go places because we have something
available for him that's going to be good for his posture but also is
functional in like society to be able to get like just even around the
grocery store. So I think the long answer or the shorter answer to
that would be we seen an occupational therapist because it allows
us to make sure that he's getting the postural support that he
needs in all aspects of his life.

Gabriel uses a round Jelly Bean switch that is about 2.5 inches in
diameter. This type of switch is recommended for users who can
access a small target area. His hands are usually fisted, so the
surface area of the switch works pretty well, but he could use
additional practice. Gabriel uses a tray on his adaptive stroller with
the single Jelly Bean switch. The switch can be moved around if
needed. There has been some concern about switch placement
lately.
