CONDUCT COMPETENCY ASSESSMENT SCRIPT


I. ESTABLISH CONTEXT OF ASSESSMENT
Yesterday, I visited the assessment center and found that all tools, materials,
and equipment that we will be using for today’s assessment are complete,
functional, and in good condition.
Today, I received the following competency assessment tool from the AC manager:
3 copies of attendance sheets
10 copies of competency assessment result summary
10 sets of rating sheets
10 copies of specific instructions for the candidates
1 copy of assessors guide
A while ago, you had your biometrics taken for the national certificate that you
will receive after the assessment when you are found to be competent.

II. SELF-INTRODUCTION
Good day, everyone!
Welcome to __________________________________, your accredited assessment center
for ______________________NC II.
I am________________, an accredited competency assessor for the qualification of
____________________ NC II. I have here my ID with my accreditation number.

III. ACKNOWLEDGE THE PERSONNEL


I would like to acknowledge the presence of our AC manager, _____________, and
_____________, our representative from TESDA provincial office, good morning
ma’am, good morning sir!
You don’t have to worry about their presence, they are here to oversee the conduct
of the competency assessment and to check if the conduct is in accordance with the
prescribed competency assessment methodology, and procedure. They will not assess
you; I will be the one to assess you.

At this moment, I am going to check your attendance. As I call your name, please

VII. PURPOSE OF ASSESSMENT


The purpose of this assessment is one for employment. For promotion, is it
possible? For advancement and upgrade, and even licensing and of course the
purpose of this assessment is to determine the level of your competency in this
qualification.

VIII. TASK TO BE PERFORMED, TO BE COLLECTED AND ASSESSMENT PROCEDURES TO BE


UNDERTAKEN
Today, I am going to collect pieces of evidence through assessment method of
written test, demonstration of skills, and oral interview. First, you will have
written test, then proceed in demonstration of skills. After the demonstration,
you will undergo oral questioning. These assessment methods can help measure your
underpinning knowledge and skills.

IX. ROLE OF ASSESSOR


As an assessor, I am here to gather evidence through observation, written test,
demonstration, and oral questioning. I have the right to stop the assessment in
case there are unwanted circumstances in the conduct of competency assessment.
Also, I will not interrupt you during the demonstration, I will simply observe.

X. NEEDS OF THE CANDIDATES TO BE CONSIDERED DURING THE ASSESSMENT


For your personal needs, the lavatories are located at the end of the hallway and
you can take your meal at the canteen, which is located near the gate.
However, during the assessment, you are not allowed to leave the premises of the
assessment center.

XI. ALLOWABLE AND REASONABLE ADJUSTMENTS WHEN APPLICABLE


By the way, if you have difficulty understanding the instructions, please do not
hesitate to inform me because I am more than willing to translate it to your
vernacular dialect and if I can’t will use Filipino language instead, so don’t be
shy to approach me okay?
Is there any of you who take medication? If none, all of you are health and in
condition to showcase your skills to pass the assessment to have national
certificate… which is great…

XII. DO’S AND DON’TS DURING ASSESSMENT


If you have concerns, questions or clarification regarding this matter concerning
in our assessment you may raise your hand because I am more than willing to assist
you. But once the assessment has started, please refrain from asking questions
because it is no longer allowed.
Using gadgets during the assessment is strictly prohibited and do not capture any
confidential documents.

XIII. RIGHTS TO APPEAL


No one is forcing you for this assessment. It is your free will to submit yourself
for an assessment. You have the right to appeal if you have doubt about the result
of the assessment and you can address it to the TESDA representative or to the AC
manager within the day because if not, you will no longer entertained.

XIV. ISSUANCE OF NATIONAL CERTIFICATE OR CERTIFICATE OF COMPETENCY


For the candidates to be found competent, the issuance of Electronic National
Certificate, you will receive it within 24 hours after the assessment via email.
It has the validity of five years and you can renew it on or before the expiry
date.

XV. REASSESSMENT PROCEDURE


After the assessment and you found to be not yet competent, don’t worry because
you can be reassessed anytime at your own convenience. In case the reassessment is
in full qualification, you have to undergo the whole assessment process. If you
are found to be competent in some units of competencies, the reassessment focuses
on the units of competencies where you are found to be not yet competent.
Also, if you are found to be not yet competent within two consecutive assessment
the, you will advise to undergo refresher course before applying for reassessment.
You can visit and coordinate with any TESDA-accredited assessment centers for your
reassessment.

XVI. CONFIDENTIALITY OF ASSESSMENT OUTCOMES


When it comes to the assessment results, we must observe strict confidentiality.
The information of your assessment result is only available to the authorize
personnel and to the institution where you have your actual training. The
assessment package is also kept in secured place to guarantee its integrity. In
this line, may I ask the candidates to refrain from giving away any form of
information that took place during the assessment procedures.
This premises are equipped with CCTV cameras to maintain integrity of the conduct
assessment. You have signed the consent form, an indication that you understand
and agree to this data collection. Please be inform that recordings and other
personal information gathered from you will be treated under Privacy Act.

XVII. OCCUPATIONAL HEALTH AND SAFETY REQUIREMENTS


As you can see, all the materials needed from this assessment are already provided
by the training center.
During the assessment, please observe safety. Make sure to check on your
electrical equipment before plugging it to avoid pulling the cord as we unplug it.
Our first aid kit is located near the door. In case of accident, please call my
attention so that we can immediately give the appropriate first aid.

During the demonstration of your skills, if you found out that you perform in an
unsafe condition I will automatically stop you from assessment.

XVIII. DISTIRBUTION OF THE SPECIFIC INSTRUCTION TO THE CANDIDATE


Do you have any questions? If none, I will now distribute to you the specific
instructions. You have 5 minutes to read the document and please do not hesitate
to ask questions right now. If you need assistance, I’ll be more willing to assist
you.

XIX. INSTRUCT CANDIDATES TO PROCEED TO THE ASSESSMENT AREA


I will give you now your assigned number. You’re candidate number 4 ma’am, and
you’re candidate number 9, sir.
So, everybody must be ready now, but, before we proceed to the assessment area let
me give you some instruction. You have there all the materials, tools, and
equipment required to complete the task and please do observe OHS protocol
After your demonstration, I will ask you some questions related to your
demonstration. You may call me when there is a need for me to translate or explain
items for clarification. After the oral questioning, I will you feedback about
your performance.
Candidates, you may now proceed to the assessment area. This way, please.

XX. ASSIGNING OF WORK STATION


Now, candidates you will have first your written test for 30 minutes. Please write
your name and indicate the set you are assigned to.
Time is up! Kindly, hand me over your answer sheet together with the test
questionnaire. I will just check it for a minute.
As I checked your written test, congratulations to everyone all of you may proceed
to demonstration of skills.

XXI. PROCEED TO THE ASSESSMENT AREA


Any questions/clarifications? If there are no more questions, you may start now.
(Assessor set the stop watch)

(Assessor will check the candidates’ performances using the rating sheet.)

XXII. AFTER THE TIME


Candidates, your time is up. Kindly hand me over your outputs and you can proceed
to the waiting area and wait for your turn for the oral questioning.

XXIII. ORAL QUESTIONING & FEEDBACKING

great because it shows that you really know what you are doing. But I observed a
while ago, you got confused about what you’re doing, right? Nevertheless, the
totality of your performance is great. Based on the rules of evidences, dimensions
of competencies and to all the evidences I have gathered during your assessment,
congratulations, you are COMPETENT in ___________________________ NC II.
Please sign the rating sheet now and you will receive your e-certificate within 24

demonstration of skills and answer all the questions in oral questioning.
Actually, I admire you for that, but hence you are here to show me your skills and
knowledge to have that National Certificate that I know you can have it but
because of your nervousness you get confused on what to do right? So, what do you
think you can do next time?
