8 Witness Point (W): An inspection activity that requires the Contractor and/or Owner and/or Third party be notified by the Manufacturer/ Vendor/ Contractor/ Subcontractor of a specific time and place for the activity to be attended. This advance notification shall be carried out at a duration specified within contract and or prior agreed with Owner by a
Formal notification process and the activity may proceed without the Contractor and/or Owner and/or Third-party representatives in case they are not available at the appointed inspection time. Still signature is required on checklist and on the Request for Inspection (RFI). No signed waiver required for Witness point.
9 Hold Point (H): An inspection activity that requires the Contractor and/or Owner and/or Third party be notified by the Manufacturer/ Vendor/ Contractor/ Subcontractor of a specific time and place for the activity to be attended. This advance notification shall be carried out at a duration specified within contract and or prior agreed with Owner by a Formal
notification process and the activity shall not
be proceeding without the Contractor and/or Owner and/or Third-party presence and written approval received by means of Request for Inspection (RFI) or an advance signed waiver has received.
10 Surveillance (S): A general monitoring and supervision of activity at any stage of the work and/or test in progress in which the Contractor and/or Owner and/or Third party verify by random attendance without any formal prior notice whether or not materials, items or works and/or tests meet the specified requirements. No formal inspection notification is
required.
11 Review (R): An activity designated by the Contractor and/or Owner and/or Third party that requires the review, approval and acceptance of the Manufacturer/ Vendor/ Subcontractor’s documentation (i-e QMS, PQ, Method of Statement, Testing procedure, Checklists, HSE Risk Assessments, design, drawings, data, materials, technical personnel
qualification, equipment calibration records and/or
test records etc.) No formal inspection notification is required but shall be submitted as document through formal agreed document management procedure.
12 Spot Witness (SW %): An inspection activity that requires the Contractor and/or Owner and/or Third party be notified by the Manufacturer/ Vendor/ Contractor/ Subcontractor of a specific time and place for the activity to be attended. Spot witness is always suffix with any no e.g 10, 20 or 50 which shows percentage of the quantum of total scope. The
selection of the percentage can be made by the
Contractor and Owner can also advise. This advance notification shall be carried out at a duration specified within contract and or prior agreed with Owner by a Formal notification process and the activity may have developed without the Contractor and/or Owner and/or Third-party representatives in case they are not available at the appointed inspection
time. Still signature is required on checklist and on the Request for Inspection (RFI). No signed waiver required for Spot Witness point.
Notes:
All reference and verifying documentation shall be to the latest revision. The inspection proportion & method shall be adjusted according to the Owner techincal specifications and relevant local & International standards.
The Format of record will refer to inspection and test record with latest version and shall be submited for owner review and approval.
