1.0 PURPOSE:

Purpose of this procedure is to provide a practical frame work to ensure

a. Safe erection and dismantling of scaffold


b. Strict compliance with scaffold standards and specifications
c. Provide safe working environment while working at height
d. To establish the guide line for the protection of people who works on scaffold
surface

2.0 SCOPE:

The scope of this procedure covers the following

a. All CCC employees including those who are working at site / location which are
under the control of KNPC
b. All sub contractors working for CCC or on behalf of CCC

3.0 REFERENCE:

a. OSHA 1910.28 Safety Requirement for scaffolding


b. OSHA 1926.451 Scaffold general requirement
c. OSHA 1926.452 Additional Requirement applicable for specific type
of scaffolds

4.0 DEFINITIONS:

Scaffold is a temporary frame work used to support people and material in the
construction or repair of building and other large structure where is required to be
performed at heights which cannot be reached from the ground.

Base Plates is a scaffold component located on the foot of a pole or frame to assist
in stabilizing the scaffold.

Guardrails are vertical barrier, consisting of top rails, and posts, erected to prevent
employees from falling off a scaffold platform or walkway to lower levels.
Toe Board is a barrier fixed at the edge/side of a platform/walkway to prevent the fall
of materials or persons from it.

Bearer (putlog) means a horizontal transverse scaffold member (which may be


supported by ledgers or runners) upon which the scaffold platform rests and which
joins scaffold uprights, posts, poles, and similar members.

Body harness means a design of straps which may be secured about the employee
in a manner to distribute the fall arrest forces over at least the thighs, pelvis, waist,
chest and shoulders, with means for attaching it to other components of a personal
fall arrest system.

Brace means a rigid connection that holds one scaffold member in a fixed position
with respect to another member, or to a building or structure.

Component person means one who is capable of identifying existing and


predictable hazards in the surroundings or working conditions which are unsanitary,
hazardous, or dangerous to employees, and who has authorization to take prompt
corrective measures to eliminate them.

Coupler means a device for locking together the tubes and coupler scaffold.

Maximum intended load means the total load of all persons, equipment, tools,
materials, transmitted loads, and other reasonably anticipated to be applied to a
scaffold or scaffold component at any one time.

Mobile scaffold means a powered or unpowered portable, caster or wheel-mounted


supported scaffold.

Personal fall arrest system means a system used to arrest an employee’s fall. It
consists of an anchorage, connectors, a body belt or body harness and may include
a lanyard deceleration device, lifeline, or combinations of these.

Platform means a work surface elevated above lower levels Platforms can be
constructed using individual wild planks, fabricates platforms.

Landing means a platform at the end of a flight of stair / ladders.

Walkway means a portion of a scaffold platform used only for access and not as a
work level.

Runner (ledger or ribbon) means the lengthwise horizontal spacing or bracing


member which may support by tubing erected with coupling devices connecting
uprights, braces bearers, and runners.

5.0 MATERIALS:

All scaffold materials shall confirm to the relevant British/OSHA standard or


equivalent
5.1 Tubes and Fittings

Type 3 and 4 steel tubes to BSEN 39:2001 is of steel with 48.3mm outside dia
/ 3.2mm thick or 48.3mm outside dia/4.0mm thick. Tubes and fittings shall be
free of crack, surface flaw or other defects. Tube shall be straight and
maintained in good condition. Pipes and fittings shall be regularly inspected
and moving part shall be lubricated.

5.2 Metal Board

Metallic board used for scaffolding deck shall be 50.8mm thick and not less
than 229mm wide. Board which are split, decayed or wrapped not be used.
Board shall not be painted or treated for in any way that may conceal the
defects.

6.0 TYPE OF SCAFFOLDS:

Following are the different type of scaffold commonly used in refinery

6.1 Independent Tied Scaffold

An independent tied scaffold is the most common form of access scaffolding


and is divided in to three group

a. Light duty independent tied scaffold


b. Medium duty independent tied scaffold
c. Heavy duty independent tied scaffold

Independent tied scaffold consist of a double row of standard connected


together longitudinally with ledger and transom at right angle to the ledger.
Braces and ties are essential for stability.

6.2 Tower Scaffold

Tower scaffold is an independent which is not tied to a permanent structure.


Stationary tower scaffold height shall not exceed more than 60 feet or 4 times
minimum base dimension. In any case the ratio of 1:4 is not possible to
maintain, the tower shall be stabilized by guying or other means.

Mobile tower scaffold consist of four or more standards standard connected


together longitudinally with ledger and transom at right angle to the ledger,
forming a rectangular scaffold mounted in wheel. Mobile tower height shall not
exceed 40 feet or three times the minimum width of base.

6.3 Cantilever Scaffold

Cantilever scaffold is a form of light independent scaffold which is entirely


depends on a structure for stability. Cantilever scaffold shall have only one
working platform with maximum height of 40 feet from truss to working
platform.
6.4 System Scaffold

System scaffold is also known as unit or frame scaffolding and is composed


wholly or partially prefabricated section. Construction is same as other type of
scaffold. Adjustable bases should provide and diagonal bracing shall be
provided for bracing. Standard scaffold tubes and couples may be used for
bracing.

6.5 Fabricated Tubular Frame Scaffold

Members shall support own weight and at least 4 times maximum intended
load. Diagonal bracing shall be used to secure vertical members.

6.6 Bracket Scaffold

Bracket scaffold are used for tank construction and maintenance. Metal
bracket and strap shall be able to withstand own weight and 4 times the
maximum weight. Horizontal strapping of strap shall not exceed 8 feet.
Bracket shall be vertical and maximum allowed at a time between two
brackets.

6.7 Suspended Scaffold

A scaffold suspended at a fixed height either below the load bearing


protecting bracket or a beam or from a structural members of roof or over
head structure. The suspended scaffold should have only one working
platform.

7.0 CONSTRUCTION OF SCAFFOLDS:

Following are the general requirements for the construction of any type of scaffold

7.1 General

Members shall support own weight and at least 4 times maximum intended
load.

7.2 Foundation, Base Plate and Sole Plate

The footing or anchorage for scaffold shall be sound, rigid and capable of
carrying intended load without setting of displacement.

8.0 SCAFFOLD INSPECTION, TAGGING & RECORD KEEPING:

All scaffold materials shall confirm to the relevant British/OSHA standard or


equivalent
1. INSPECTION
The jobsite should be inspected and supervision be familiarized with proper access,
proximity of power or process lines, obstructions, ground conditions, openings or
pits, strength of supporting structure, interference with other workers, overhead
protection, wind / weather protection and environmental hazards. These conditions
must be evaluated and adequately provided for. Also, consider the protection f
people who will be passing or working beneath or around scaffold structures.

All equipment must be inspected to ensure that it is in good condition. Damaged or


deteriorated equipment should not be used and must be removed from service.

Each scaffold and scaffold component must support without failure its own weight
and at least four times the maximum intended load applied or transmitted to it.
Scaffolds and its components must not be loaded in excess of their maximum
intended loads.

2. WOK PERMIT

Concerned supervisor must obtain a work permit and ensure its strict complains.
Work permit shall always be available at work site. No work should be carried out
without work permit. At the end of shift, permit holder must close the permit and
submit to concerned authority or should follow the instruction of permit issuer as
directed.
3. INDUCTION TRAINING
All employees must be included before commencement of project work by the safety
representative. They must be aware of assembly point, emergency evacuation,
emergency alarm, emergency telephone no., use of PPEs, prohibited items at work
site, safety at work etc.

4. RESPONSIBILITIES
Responsibility for the inspection of scaffolding and other temporary means of
access is assigned to a Safety Officer of Assignee.

Scaffolding supervisor is responsible for erecting, alerting or dismantling of the


scaffolding and other temporary means of access. His duties include inspection and
tagging of structure prior to handover to the user.
a. SCAFFOLDING ERECTION SUPERVISOR
An important function of the supervisor is to verify that all scaffolding released for
work to other departments is properly tagged. These tags, green in colour, are only
attached after a scaffold has been completed and inspected by the supervisor or a
designated scaffolding foreman. No other departments are allowed to attach a
green tag and access to scaffold.

Safety representative acts in monitoring capacity. He has the authority to remove
green tags and barricade a scaffold to prevent access if scaffold is unsafe for use.
His role is not only to monitor the scaffold, but to observe any interference to
existing scaffold from other ongoing activities.
5. GENERAL REQUIREMENTS AND PRCAUTIONS:

 A sole plate shall be provided to spread the load on earth if the soil / ground are
loose of not strong enough to take the point load of scaffold.
 Base plate shall be provided for all scaffold
 Bracing shall be provided for the scaffold and it should always remain in position
 All decking shall be provided for easy and safe access and all ladders and steel
boards shall be free from oil, Greece etc.
 During erection and dismantling of scaffold, proper work procedure shall be
implemented and all the activities shall be performed under proper supervision
 During erection of pipes on pipe rakes, pipes should not be supported on scaffold
and chains or pulleys shall not be used on scaffold to load or unload the material
 Near the road crossing or main access the suspension type scaffold shall be
erected.
 Employees must be provided with fall protection devices
 Always maintain the checklist of scaffold and ensure that it is safe to use.
 Never allow any other persons to do the alternation / modification on the scaffold,
only trained / authorized scaffolders are allowed to do the same.
 Ensure that ladders are properly fitted.
 Never climb the ladder one handed. Always firmly grip with both hands.
 Never walk on slippery gangways, platforms stair etc.
 Always train the workers on fall hazards
 All people involved on erection and dismantling of scaffold shall be properly trained
on their job and should use proper personal protective equipments. During
erection/ dismantling of scaffold the area must be barricaded to prevent
unauthorized entry and a sigh board must be place to ward others.
