1. Has the area where the line cutting will occur been properly identified excavated if
underground, cordoned off, and proper personnel notified? Yes No

2. Is approved Drawing available & indicating the cold cut location? Yes No

3. Is the line to be cut and the cut location clearly marked and varified by the
Contractor’s Engineer/ Site Engineer and communicated to permit issuer?

5. Has the area where the line cutting will occur been properly identified and NDT

points identified and positively isolated blinds with a gasket are installed at each
flanged termination and interconnection point next to the isolating valve or flange?

the line been disconnected(Lock out/ Tagout) with visible marking? Yes No

8. Has the line been depressured completely and physically verified? Yes No

11. Was the line tested for combustibles or toxic content? Yes No
