that on or about 8:00 AM of December 2 , 2021,PLTCOL EDGAR E AZOTEA,


Chief of Police, conducted Inspection, Accounting of Personnel, Dissemination
of directives and memoranda from the higher headquarters, and Daily Police

and children, family and domestic relations counseling, effective
parenting and Early Childhood Care Development, GST, NAPWPS and Safe
Spaces Act. Furthermore, personnel were directed to observed personal
security and be vigilant at all times and also strictly implement the COVID 19
Health Protocols.

That at about 9:00 AM of this date, team led by PEMS MA. LEAH B BITARE,

Duty OD, conducted regular patrolling and checking of traffic/patrol personnel

At about 9:30 AM, team led by PMSg AmielB Binalyo, Traffic/Patrol PNCO
under the supervision of PLTCOL EDGAR EAZOTEA, Chief of Police,
conducted Inter-agency coordination/dialogue/ pulong-pulong/ establishment
security inspection to the security guard of LCC Department Store, A. Berces
Street, Brgy. Basud, Tabaco City regarding observance of the minimum
health standard protocols, wearing of face mask/face shield, reminded
them on how to prevent future untoward incident and be vigilant in their personal
security against lawless elements. Likewise, said personnel also explained EO

9165 (Comprehensive Dangerous Drugs Act of 2002),Crime Prevention and

At around 02:30 PM of November 10, 2021 personnel of this office under the
supervision of PLTCOL EDGAR E AZOTEA, Chief of Police of Tabaco CPS No untoward

point of origin (Brgy. San Antonio) going to their respective destination (Tabaco reported

That at around 3:00 PM traffic/patrol personnel led by PEMS MA LEAH PADRE
conducted OPLAN SITA along City Poblacion with three (3) apprehension for Properly

Nature of the Case : Violation Art II Sec 5 & 11 of RA 9165 (DRUG
BUY-BUST Operation)

DTPO : 2:55 PM of December 2, 2021 at P-2 Brgy Sto. Cristo,


Tabaco City

Suspect/s : MESTICO TUSCANO BORAL aka “TIC”, 25 years old


(DOB:
October 22, 1997: POB: Tabaco, Albay, Single (WLP),
jobless and a resident of P-2 Brgy Sto. Cristo, Tabaco City.

Status : Arrested and presently detained at the custodial facility of this


Station

On aforesaid time, date and place of incident, CDEU team of


Tabaco CPS led by PCMS ANICETO G COLARINA, Intel PNCO together
with PIU Albay PPO, Albay Maritime Police Station under the over-all
supervision of PLTCOL EDGAR E AZOTEA, Chief of Police, in
coordination with PDEA RO5, conducted Drug Buy-Bust Operation that
resulted to the arrest of the above named suspect after selling one (1) pc
medium size heat sealed transparent plastic sachet containing white
crystalline substance suspected as shabu to the poseur buyer with the
help of confidential informant and confiscation of the following items, to wit:

i. One (1) pc color yellow and black cotton coin purse


marked as JJB-1 with signature;
For filling
ii. Cash Money amounting to Four Thousand Pesos thru inquest
(Php 4,000.00) composed of One (1) pc genuine proceeding

One Thousand peso bill with serial nr G549020 and at the office
Three (3) pcs One Thousand Pesos boodle money of the City
with same serial nr as FL776651 USED AS BUY- Prosecutor
BUST MONEY;

iii. One (1) pc matchbox FUEGO marked as JJB-2 with


signature – all of which were found at the right front
pocket of the suspect short; and

iv. Three (3) pcs small size heat sealed transparent


plastic sachet containing white crystalline substance
suspected as “shabu” marked as JJB-3 with
signature, JJB-4 with signature and JJB-5 with
signature- found inside the matchbox.

The search, markings, collection and inventory of confiscated items


were done at said place in the presence of the suspect, other Police
Officers and mandatory witnesses namely; elected Barangay officials,
Brgy Kgwd Vincent B Barbacena, Brgy Kgwd Danilo B Manlangit;
Media Representative Mr Rodel B Balingbing, and NPS Representative
Mr. CALIXTO C BOGÑALBAL. The inventory ended at around 3:50 PM of
same date. The affiants in this case are Pat Jonel Jay B Bien, acted as
Poseur Buyer/Arresting/Seizing Officer while PCpl Christian Paul P
Firaza, who acted as the Back-up arresting and PSSg Christian B Romano
as the official videographer. Prior to the search of the arrested suspect,
Pat Bien submitted himself to Brgy Kgwd Vincent B Barbacena to be
searched and checked to ensure the transparency of the search against
the arrested suspect.
4. Action Taken:

The suspect was informed of the nature of his arrest and apprised
of his constitutional rights in Bicol dialect known to and understood by him.
Likewise, he was subjected for mandatory medical examination at Ziga
Memorial District Hospital, Tabaco City assisted by Pat Bien.

5. Remarks:

Cases for Violation of Section 5 and 11 Article II of RA 9165 are


being prepared by this station for filing/referral thru Inquest Proceeding.
Progress report will follow.

6. Investigator-on-case: PCpl Gear T Balute – 09383402920.

That at around 10:00 PM, team led by PEMS PADRE, Officer-Of-The-Day


on board of mobile patrol 01 conducted area patrolling, police visibility,
accounting of personnel deployed at their area of responsibility and strict
implementation and observance of the standard health protocol at City
07 Poblacion especially wearing of face mask. Likewise, initiated inspection of Favorable
government and private edifice and reminded to observe curfew hours. result
Further, team conducted mobile patrolling along sea port, sea wall and
enforce curfew hours especially to minors and senior who still outside of
their residence and reminded them to observe curfew hours.
That at around 11:00 PM team led by PEMS PADRE, OD conducted
patrolling along San Carlos, Pawa, Brgy Bombon, Bangkilingan, Panal,
Matagbac, Guinobat, San Antonio, San Vicente, Brgy Mariroc, Bognabong,
Pinagbobong, Guinobat, Basud, Barangahawon, San Ramon, Cobo,
No untoward
08 Cabagnan, Fatima, Quinale, Sto Cristo, Cormidal, San Roque, Salvacion, incident
Fatima, Divino Rostro, Quinale, San Juan, San Lorenzo and Pawa this reported
City. OD conducted dialogue with the duty Brgy officials and Tanod their
respective Brgy Hall. Further conducted inspection of all vital installation,
bridges, rivers, sea wall, sea port and terminal within this City.

ALL WALK-IN COMPLAINANT AND INCIDENTS ARE PROPERLY RECORDED


AND ACTED UPON BY THE DUTY ON POST PERSONNEL. LIKEWISE, ALL
DUTY PERSONNEL/ SENTINEL PROPERLY TAKE THEIR POST.
