INSTRUCTIONS TO THE CANDIDATES FOR UPLOADING DOCUMENTS

Dear Candidate,

Read the following instructions carefully and upload all the relevant documents as applicable.

1. Scan the Photograph of size 3.5x4.5cms in (JPEG/JPG) and the file size should be 50 to 300 kb.

2. Scan the documents on a color scanner as per the prescribed file size and image type (JPEG/JPG/PDF) and upload. In case of documents
with one page, the file size should be 50 to 300 kb. The documents with more than one page should be uploaded as a single file in PDF format
with size of 50 to 500 kb.

3. In the document verification portal, relevant certificates enlisted therein are to be uploaded. Preview for each uploaded document has been
provided. Candidates should ensure that the images/documents are correctly uploaded before submitting. Candidates should also note that
mere saving of the uploaded images in the portal does not complete the uploading process, unless it is followed by SUBMIT. In case an
uploaded document(s) is found to be incorrect/not readable during checking the preview, then the correct document(s) can again be
selected/scanned and uploaded till the last date of uploading documents.
4. Only latest passport size color photograph taken within the last ONE month should be uploaded.

5. If Father's or Mother's name, as indicated in the application, is not available in the Matriculation certificate, then scanned image of Transfer
Certificate from the Institution last attended, indicating Father / Mother name should be uploaded by using the provision to upload additional
certificates.

6. Certificate or Provisional Certificate for the prescribed Educational Qualification has to be uploaded in the portal. Ensure that the certificate
date is within the last of submission of application against CEN 3/2019. In case, the certificate date is after the last date of application
submission, then the candidates need to upload all the semester wise mark sheets also as a single PDF file by using the provision to upload
additional certificates.

6. The uploaded Caste certificate for SC/ST should be as per the prescribed format given in Annexure-I of CEN 3/2019 with the applicable
community clearly ticked. In case of variation in the name of candidate or father's/mother's name in the certificate, candidate should get a new
caste certificate with correct name(s) as mentioned in SSLC/Matric Certificate.

7. The uploaded Caste certificate for OBC-Non Creamy layer candidates should be as per the prescribed format given in Annexure-II of CEN
3/2019 and issued by Competent Authority. The certificate should not be older than one year from the date of Document Verification. In case of
variation in name of candidate or father's/mother's name in the certificate, candidate should get a new caste certificate with correct name(s) as
mentioned in SSLC/Matric Certificate.

8. In case of Ex-Servicemen candidates, Declaration as per the prescribed format in Annexure - VII of CEN 03/2019, relevant sheets of the
Discharge Certificate containing name, date of appointment, date of attestation, date of discharge and Ex-Serviceman status should be
uploaded as a single PDF file.

9. Candidates belonging to Economically Weaker Section (EWS) should upload the INCOME & ASSEST certificate as per the prescribed format
in Annexure - III of CEN 03/2019. Any such Certificate issued even after date of notification in 2019 will also be considered as Valid.

10. Candidates belonging to Economically Backward Community (EBC) who have sought waiver of examination fee should upload Income
certificate as per the prescribed format in Annexure - IIIA of CEN 03/2019.

11. Minority Community candidates who have sought waiver of examination fee should upload the Minority Community Declaration on Non-Judicial
Stamp Paper as per the prescribed format in Annexure-IV of CEN 03/2019.

12. PWD candidates have to upload Disability certificate as per the prescribed formats in Annexure V(A) or V (B) or V (C) whichever is applicable.

13. Serving Railway Employees / Other Government Employees are required to produce 'No Objection Certificate' (NOC) issued by the Competent
Authority duly indicating the Date of Appointment.

14. Upload Affidavit for minor variations in Candidate's or Father's / Mother's name like spelling error, initial or surname sequence in certificates in
PDF format in the provision made to upload affidavits. However, such affidavits will not be accepted for variations in names in caste
certificates.

15. Female candidates whose name has changed Post marriage should upload Gazette Notification or any other legal document as applicable.

16. In additional documents table, documents like semester wise mark sheets and ANY other relevant additional documents, if any should be
uploaded.
Candidates must bring the following original certificates / documents (as applicable in their case) along with e-call letter:-

1) Matriculation/High School Examination Certificate or Equivalent certificate as Proof for Date of Birth and Matriculation
Qualification. (The candidates’ name and the father/mothers’ name mentioned in the application will also be verified with
reference to the names mentioned in this certificate.)

2) +2 / Inter / Higher Secondary/PUC/Equivalent Certificate (As applicable)

3) A University degree or its equivalent

4) SC/ST certificate in the format as per Annexure I

5) OBC-NCL certificate as per Annexure II (Not older than one year from the date of DV) & declaration as per Annexure IIA

6) Non creamy layer declaration by OBC-NCL candidates as per

7) Income and Asset Certificate to be produced by Economically Weaker Sections as per Annexure III

8) Income Certificate for Waiving Examination Fee for Economically Backward Classes as per Annexure III A

9) Minority Community declarations on Non-judicial Stamp Paper as per Annexure IV

10) Original discharge certificate for Ex-servicemen.

11) Medical Certificate for Persons with Benchmark Disabilities (PwBD) as per Format at Annexure V(A)/ Annexure V(B)/
Annexure V(C)

12) Self-declaration for Ex-servicemen as applicable. Annexure VII

13) NOC from serving employees with date of appointment from current employer.

14) NOC from Serving Defense Personnel (ExSM) with probable date of discharge.

15) Gazette notification and/or any Legal document in case of formal change of name as mentioned in Para 1.7.

16) Receipt of payment if made through a challan at SBI branch or Pay-in-slip at computerized Post Office.

17) Self-Certification by the Transgender candidates

18) J & K domicile certificate, if applicable.

19) Certificate of eligibility has been issued by the Government of India, as per Para 4.1 (f).

20) Decree of divorce/judicial separation from the competent court of law as applicable and Affidavit stating that the candidate has
not remarried.

21) Death Certificate in case of widow and Affidavit stating that the candidate has not remarried.

22) ExSM candidates who secure civil employment after applying for this CEN should give self declaration to the concerned
employer about the details of application against this CEN as soon as they join. The acknowledged copy of this declaration
along with NOC from the civil employer should be produced during DV.

23) Passport size colour photographs NOT MORE THAN ONE MONTH OLD.

24) In case of variation in spelling in name, fathers name / mothers name in the on-line application and original certificates
/documents, candidates are required to submit relevant affidavit executed before Magistrate or Notary on required stamp
papers stating that the referred person in certificate(s) is one & same on the date of verification.

25) Aadhar Card / e-Aadhar card.

26) Any other relevant certificate /testimonials/documents available with the candidate.

27) The candidate should also bring two(2) sets of clear & legible self-attested photocopies each of the original certificates /
documents mentioned above against (1) to (26)(As applicable) in A4 size Paper.

28) 3 passport colour photograph not more than one month old.
