AFFIDAVIT

I, MRS. MUMTAZ MAINUDDIN SAYYED, Age : 65 years,


residing at – 403, Khatri Apartment, C-wing, Khurshid
Compound, Rabodi No.1, Thane West : 400 601. do hereby state
and declare on solemn affirmation as under:-
1. I the undersigned resident of above mentioned address

states that I was born on 14th September, 1949 at Mudhol,

Dist. Bijapur, State : Karnataka.

2. I say that my above mentioned date of birth is as per my

school leaving certificate.

3. I say that my date of birth stated in the my school leaving

certificate is taken from my birth certificate.

4. I say that I am making this affidavit to enable the Income

Tax Department to ascertain my age and to issue me PAN

Card.

Whatever stated hereinabove are true and correct to the best of

my knowledge, belief and information.

Solemnly declared at Thane on 3rd day of July, 2015.
