Arvind Uttarkar was nominated by his mother as the nominee for the flat and shares. 3) Arvind Uttarkar has applied to transfer the flat and shares into his name. Vandana states that she has no objection to this transfer.

AFFIDAVIT

I, MRS. VANDANA SUHAS SULAKHE aged 58 years, daughter of late


(Mrs.) Kalpana Atamaram Uttarkar, an adult of Mumbai Indian Inhabitant,
residing at 20A, Dhanlaxmi CHS , 3rd floor, Mogal Lane, Matunga (West),
Mumbai 400016, do hereby state and declare on solemn affirmation as
under:-
(1) I say that my mother, late (Mrs.) Kalpana Atamaram Uttarkar was
an owner of flat bearing No. 403, Ajay Shopping Centre, T.H Kataria Marg,
Mahim (West), Mumbai-400016. She was the member of the Ajay
Shopping Centre Premises CHS and the Society had issued Share
Certificate bearing No. 15, for five fully paid up shares of Rs. 50 each
bearing distinctive numbers 71 to 75 in her name.

(2) I say that my mother, Mrs. Kalpana Atamaram Uttarkar dies


intestate at Mumbai on 30th May2021 leaving behind my brother, Arvind;
sister Kavita and me as her only heirs. Mr. Arvind A. Uttarkar is duly
nominated by my mother as her nominee in respect of the said flat and
shares.

(3) I say that my brother , Mr. Arvind Atmaram Uttarkar, aged 55 years,
adult Indian Inhabitant, having his address at flat no 403, Ajay Shopping
Centre, T.H Kataria Marg, Mahim (West), Mumbai-400016 has applied
with the Ajay Shopping Centre Premises CHS Ltd. for transfer of the said
flat and the said shares in his name.

(4) I further say that I have NO OBJECTION of any kind, if the said
flat and the shares are transferred by the Society favour of MR. ARVIND
ATMARAM UTTARKAR.

(5) I state and submit that I shall not claim any right, title or interest in
the said flat and the shares, now or in future. My sister, Kavita and me ,
have already released our rights in respect of the said flat and shares in
favour of our brother, Mr Arvind Atmaram Uttarkar. I state that we have
willingly, voluntarily and without any influence on our mind have
transferred, relinquished and surrendered all our rights, title and interest in
respect of the above said flat and share in favour of our brother, Mr Arvind
Atmaram Uttarkar.

(6) I am making this Affidavit to submit the same to the Ajay Shopping
Centre Premises CHS Ltd, T.H Kataria Marg, Mahim (West),
Mumbai-400016 for the purpose of recording the NO OBJECTION in
connection with the transfer of the above referred flat and the shares in the
name of MR. ARVIND ATMARAM UTTARKAR, and I hereby
indemnify the Ajay Shopping Centre premises Co - Operative Housing
Society Limited and all its concerned office bearers and members who
will make the transfer based on this affidavit.

(7) I say that whatever is stated hereinabove is true to the best of my


knowledge and ability and I believe the same to be true.
